1,000 Sign on bonus! & Employee referral Bonus offered POSITION SUMMARY The purpose of your job position is to provide each resident with routine daily nursing care in accordance with current federal, state, and local standards that govern the facility and as directed by your supervisors. ESSENTIAL DUTIES AND RESPONSIBILITIES Ensuring that all nursing care is provided in privacy Checking vital signs Dressing wounds Inserting catheters Listening to residents and their families Addressing questions/concerns of residents Performing accurate and detailed charting of resident progress notes Timely reporting of change in resident’s condition to the nurse supervisor Assisting and supervising CNAs Ensuring equipment is in good operating order Monitoring residents that are at risk for falls Collecting lab samples Abiding with all facility policies and procedures including not disclosing user ID codes and passwords Following Infection and Control policies Reporting any occupational exposures to blood, body fluids, or other hazardous materials to a supervisor immediately Attending annual facility in-service training programs Completing other assignments when requested by management Every effort has been made to identify the essential functions of this position. Job Description The Medical Coordinator (LVN) is responsible for delivering high quality medical care and influencing positive outcomes for patients experiencing homelessness in the recuperative care setting. We have many schedules available for this role, including weekends and nights. The pay range for this position is $34.00 - 36.00 per hour, depending on experience . This position also qualifies for $1.50 swing shift differential and $3.00 graveyard shift differential . The schedule for this position is 7am to 3:30pm, 5 days on, 2 days off. Sunday - Thursday. Work site may vary based on operational needs. Responsibilities Client Care: Perform initial intake to ensure that an appropriate care plan is established based on discharge information from the hospital or primary care notes Assess health status of patient upon arrival by conducting head to toe assessment and checking vitals Determine the patient’s insurance status, make follow up appointments with patient’s providers and reviewing medication list Establish and maintain a care plan Set weekly goals for patient to complete to assist with medical or social reintegration, verify goal completion, and assess medical needs. Conduct morning, afternoon, and evening checks for all assigned clients to observe wellbeing, ensuring there is no change in medical conditions since hospital discharge.
