Description A nurse practitioner (NP) as described in Title VIII, Article 139 section 6910 of the New York Education Law is a RN who has earned a separate certification as an NP through advanced clinical nursing education in a distinct specialty area of practice. Nurse practitioners are independent practitioners who diagnose, treat, and prescribe for a patient’s condition that falls within their specialty area of practice. NOTE: Medical orders, progress notes and treatments plans are not required to be countersigned by a physician. Responsibilities CLINICAL PRACTICE 1. Assesses the health status of patients/families by means of comprehensive health history and physical examination. 2. Initiates and orders selected therapeutic and diagnostic procedures, consultations and laboratory examination, prescribes medication regimens, interprets findings and alters plan of care according to accepted standards of care and practice guidelines 3. Educates and counsels in the areas of health promotion, maintenance, disease prevention, and management of acute/chronic illnesses. 4. Documents findings and plan of care in the patient's medical record. 5. Consults with physician according to established standards, policies, procedures, and protocols. 6. Demonstrates the knowledge and skills necessary to provide care, based on physical, psychosocial, educational, safety, and related criteria, appropriate to the age of the patients served in assigned area. 7. Initiates referrals to specialty services and fosters continuity of care. 8. Participates in team meetings and conferences to enhance an interdisciplinary approach to care delivery. 9. Maintains patient/employee confidentiality in the management of information. 10. Develops the plan of care based on the diagnoses and individual patient needs: • Records, reports, and interprets the client’s responses to the plan of care. 11. Performs a patient record review with the collaborating physician in a timely fashion but not less than every three months if utilizing a collaborative practice agreement. 12. Completes the Focused Professional Practice Proctoring Evaluation (FPPE) or the Ongoing Professional Practice Proctoring Evaluation (OPPE) as required by Medical Staff for continued credentialing status. 13. Practices as a member of the allied health staff according to the rules and regulations of the Medical Staff and the bylaws of the Hospital Staff. PROFESSIONAL DEVELOPMENT 1. State Certification: Certified by the New York State Department of Education as a Nurse Practitioner with current registration as follows: Is a graduate of New York State registered Nurse Practitioner Program or equivalent OR is certified as a Nurse Practitioner by a national association via examination. Has completed a pharmacy component of not less than the required semester hours or the equivalent AND has been instructed in New York State and Federal Laws relating to prescriptions and recordkeeping (3-hour course). Has a Collaborative Practice Agreement with a physician and designated protocols if less than 3600 hours experience as a practicing Nurse Practitioner. OR Has a Collaborative Relationship with a physician based on employment in clinical practice in the NP role at Mount Sinai Hospital. If there is a documented work history as a Nurse Practitioner with greater than 3600 practice hours. The establishment of Collaborative Practice Agreement Chapter 257 of the Laws of 1988 of New York State which took effect April 1, 1989, authorizes Nurse Practitioners certified by the State Education Department to use the title "Nurse Practitioner" [for Nurse Practitioner job description, see appendix] and engage in the expanded practice of nursing, which may include the performance of primary health care services and the issuance of prescriptions. Practice agreements and practice protocols must be established and maintained in the practice settings of the Nurse Practitioner and collaborating physician where they shall be available to the Department for inspection. 2. Practice Protocols Practice protocols shall identify the area of practice to be performed by the Nurse Practitioner in collaboration with the physician and shall reflect accepted standards of nursing and medical practice. Protocols shall include provisions for case management including diagnosis, treatment, and appropriate recordkeeping by the Nurse Practitioner; and may include such other provisions as are determined by the Nurse Practitioner and collaborating physician to be appropriate. These protocols will be determined by area of specialty. 3. Practice Agreements Practice agreements shall include provisions for the following: a. Patient Assignment Patients are assigned to the Nurse Practitioner or physician according to the needs of the patient. b. Diagnostic Tests Laboratory tests may be ordered according to the initial work-up guidelines found in the reference protocol. Collaboration is not necessary for routine diagnostic procedures. However, consultation and documentation should be completed before more extensive tests are ordered, e.g., nuclear medicine tests, MRI, CT scan, IVP, etc. c. Consultation Nurse Practitioners consult with and refer patients to subspecialty clinics as needed. d. Prescriptive Practice Nurse Practitioners prescribe medications according to the New York State Laws and Regulations for prescription writing and record-keeping (see Appendix). e. Charting Nurse Practitioners are responsible for documenting findings, consultations and plans of care in the patients' medical records. A Fred Hutch Cancer Center ARNP/PA-C is a certified physician assistant or advanced registered nurse practitioner who provides evaluation and medical management of multiple service lines of adult oncology patients at our community site at Northwest Hospital. They will actively coordinate the complex care of cancer patients undergoing both systemic anti-cancer therapy and radiation therapy. The incumbent will manage side-effects of these therapies, order appropriate laboratory and radiographic studies; prescribe medications and refer patient to other specialists as necessary. The incumbent may provide for post-treatment cancer patients follow-up visits for the medically indicated period after completion of treatment. The incumbent will also provide psychological support to patients by participating in patient and/or family teaching and refer patients to community agencies and other resources to promote optimal adjustment and functioning. This position requires the ability to lead other clinical staff, be accountable for the work required in this position, the ability to be flexible in a dynamic environment, and the relentless pursuit of excellent patient centered care. This clinic provides a broad range of services including GI, Lung, GU, Breast, and Hematologic Malignancies and Radiation therapy. Responsibilities Performs complex and problem-focused H & P’s Evaluates and manages on-treatment and post-treatment side effects, as well as long-term surveillance and survivorship issues. Description The Clinical Program Manager provides clinical support and administrative direction within the Tisch Cancer Center’s Clinical Trials Office (TCC CCTO). In collaboration with the Associate Director for Research, clinical trials investigators, and TCC CCTO Administrative Directors; this candidate is responsible for the implementation and conduct of clinical trials with investigational anti-cancer agents in the Cancer Institute. The Candidate must have expert knowledge of cancer as a disease process, cancer treatment modalities, and the process of conducting clinical research. The Candidate will follow a caseload of patients in the ambulatory setting and assist in the administration and plan of the research program. The Clinical Program Manager will report into the CCTO’s Administrative Director. Responsibilities Administrative Responsibilities: Supervises Clinical Research Coordinator positions and other CRNs to ensure protocol compliance, accurate data collection and sample acquisition. Responsibilities ESSENTIAL FUNCTIONS Performs comprehensive history and physical examinations on patients with cancer to assess normal and abnormal findings and identifies differential diagnoses on established and new patients. Interviews, assesses, and evaluates oncology and hematology patients receiving outpatient infusion and other therapies to determine their physical condition and orders initial diagnostic tests, collects specimens for routine laboratory tests, and performs screening and therapeutic procedures appropriate for the patient’s medical condition. Writes orders for administration of blood products and chemotherapy agents under the direction and supervision of attending physician. Orders consults for services such as physical therapy, occupational therapy, respiratory therapy, imaging studies, and nursing services; and writes referrals for consultations to radiation oncology, writes orders for vascular access device placement for chemotherapy, and arranges for procedures and biopsies with the surgery and interventional radiology services. Formulates an evidence-based treatment plan according to the interpretation and evaluation of all patient data. Collaborates with other healthcare team members involved in patient treatment to ensure continuity of care. Initiates arrangements for hospital admissions and discharges patients as indicated by clinical status. Manages the clinical course of patients with cancer with acute or chronic complaints and end-of-life and other health concerns. Performs core clinical practice competencies specific to the patient population, and promotes, directs, evaluates, and manages safe patient care for patients being treated in the chemotherapy infusion area, and provides patient teaching and coordinates counseling. Prescribes medications and administers orders as indicated by patients’ clinical status. Serves as a cancer pain management resource to the medical center in both outpatient and inpatient settings. Recognizes and initiates lifesaving procedures for oncologic emergencies and brings emergencies to the immediate attention of a physician. Participates in the tumor board and clinical research studies as determined by the needs of the program and participates in staff development and in-service education as requested. Promotes and participates in the delivery of patient and family education on the cancer diagnosis, treatment, expected outcomes, and end-of-life issues such as do-not-resuscitate orders, healthcare proxies, and advance directives; and assists in the completion of patients’ disability and insurance forms as needed. Ensure proper PPE is always worn while on duty including but not limited to, face mask, gloves, gown, isolation gown, NIOSH-approved N95 filtering facepiece respirator or higher, if available), and eye or face shield. Complete all donning and doffing tasks in a safe acceptable method and discard of used PPE accordingly.
