This includes conducting thorough daily rounds, performing advanced medical history and physical assessments, formulating, and implementing patient-centered treatment plans, interpreting diagnostic test results, and facilitating safe and effective discharge planning. (20%) Orders, interprets, and evaluates laboratory, radiological, and diagnostic studies to determine patient treatment plans and establish medical diagnoses within the scope of practice. Collaborates proactively with the attending physician and interdisciplinary team to develop and refine patient care plans. Seeks physician consultation and guidance for clinical decisions that extend beyond the CRNP scope of practice or knowledge base, ensuring optimal patient safety and outcomes. (15%) Institutes emergency measures, treatment, and appropriate stabilization for acute patient situations, such as cardiac arrest, hemorrhage, or shock. (15%) Ensures appropriate and timely documentation in the Electronic Medical Record (EMR) to support accurate medical coding and billing, and record and report pertinent information to the attending physician. Ensures timely documentation of all patient encounters, interventions, and outcomes in the electronic health record, adhering to all regulatory and institutional standards for inpatient care of acute and chronic conditions. (10%) Facilitates seamless communication and coordination of patient care activities with patients, families, community health services, and the entire interdisciplinary healthcare team to ensure continuity of care and optimal patient experience. Assesses the specific learning needs of patients and their families, delivering culturally sensitive and age-appropriate education regarding their condition, treatment plan, and self-care strategies to promote understanding and active participation in their recovery. (10%) Communicates effectively with the attending physician regarding the plan of care, involving them in clinical decision-making for issues outside the Nurse Practioner's knowledge or scope of practice. (10%) Maintains mandatory professional continuing education and actively participate in patient safety, quality improvement processes, and patient engagement initiatives. Summary The Section of General Internal Medicine at Baylor College of Medicine is seeking a Nurse Practitioner to provide services at CHI St. Luke's The Woodlands Hospital. The ideal candidates are patient-centered and committed to excellence in providing quality care. Job Duties Effectively identifies, evaluates, and addresses acute and chronic problems as well as disease prevention and health promotion issues of the patients while administering quality patient care. Works in an independent and interdependent relationship with the physician, which allows for consultation, collaboration, or referral. Responsible for initial consultation and/or follow up diagnosis and treatment of acute and chronic healthcare issues of clinic patients. Responsible for communicating test results and recommendations to patients and/or their caregivers. Responsive to the questions and concerns of patients and their family members. Educates patients and/or families about preventative care, medical issues, and use of prescribed medical treatments and or medications. Maintains accurate and confidential medical records. Documents all medical evaluations, diagnoses, procedures, treatment, outcomes, education, referrals, and consultations consistent with state regulatory standards. Facilitates evaluation of records by physicians and peers and receives and implements constructive directives. Prepares cardiac clearance letters to referring physicians, for transplant and pre-operative clearance. Prescribes medications in accordance with prescriptive authority granted by the Texas Medical Board/Board of Nurse Examiners. Maintains professional certification as dictated by the Texas Medical Board/Board of Nurse Examiners. Follows latest ACC and AHA guidelines as protocol and standard of care which shall be defined to promote the exercise of professional judgment by the NP/PA commensurate with his/her education and experience. Review billing cases log Demonstrates a positive attitude in the workplace and is a leader in this regard. Provides follow up patient care within a private practice setting. Conducting rounds, evaluating patients returning from off-site treatment Admitting and discharging patients from the facility Qualifications Education / Experience / Requirements Advanced nursing education (Master's or Doctorate) Current licensure as a Nurse Practitioner Must hold a current unrestricted license as a Nurse Practitioner in Arizona Current DEA registration An appropriate collaborative agreement with the on-site Physician Must possess an active BLS CPR Certification (American Heart Association preferred) Be certified in a Board recognized by the American Board of Medical Specialties Knowledge of nursing practice principles and techniques, occupational health and safety standards, infectious disease management, applicable health care laws, regulations, and procedures for direct health care services. Ability to maintain confidentiality of all proprietary and/or confidential information Excellent communication, organizational, and problem-solving skills Able to work independently and in a team Experience working in a correctional facility or acute care center is a plus Integrity, professionalism and ability to adhere to a Code of Conduct and comply with all facility(s) correctional healthcare policies, procedures, and legal requirements Must pass the facility’s criminal background check and drug screening Physical Capabilities: Must have the ability to stand for long periods, lift and move patients (up to 50 pounds), bend, squat, reach, push, pull, walk significant distances, and perform physically demanding tasks like CPR, all while maintaining good balance and endurance throughout a shift. Overview The Advanced Practice Provider (Nurse Practitioner or Physician Assistant) in Cardiovascular Surgery plays an integral role in the comprehensive care of adult cardiothoracic surgical patients. This provider works collaboratively with surgeons, anesthesiologists, intensivists, nursing staff, and multidisciplinary teams to deliver high‑quality perioperative care. Responsibilities include inpatient rounding, first assisting in the operating room, managing pre‑ and postoperative patients, and participating in call coverage. Overview: Serve as first assist in cardiovascular surgical procedures, including CABG, valve surgery, aortic procedures, and other cardiothoracic operations. Perform surgical tasks such as vein harvesting, suturing, retraction, cannulation assistance, and hemostasis. Support preoperative preparation and postoperative handoff to ICU teams. Perform daily rounds on postoperative and step‑down patients. Manage routine and complex postoperative issues, including hemodynamic monitoring, chest tube management, wound care, and medication adjustments. Order and interpret diagnostic tests, labs, imaging, and consults. Coordinate patient care with nursing, case management, rehabilitation, and other specialties. Participate in shared call rotation and support OR call needs.
