The Plan of Care will be updated according to policy/guidelines and as needs change. Following a patient’s death, assists with closure contact and support in collaboration with IDT. Directs and supervises the care provided by the LPN and Home Health Aide following agency policy. Evaluates services provided by the Home Health Aide every two weeks with periodic on-site supervisory visits. Demonstrates the ability to administer, monitor and document medications as prescribed by attending physician. Uses age-specific care practices and provide appropriate patient/family teaching.
