A Brief Overview There are two units within cardiac intermediate care. These units p rovide intermediate level care to patients with congenital and acquired heart disease as well as those with primary rhythm abnormalities. Patients include medical and surgical cardiac patients with an age range typically spanning newborns to young adults. Units may also care for patients under local subspecialty care or general pediatrics. Nurses will interview and be hired to one of these two units depending on position availability at time of application. The Clinical Nurse practices at the level of an advanced beginner to expert practitioner of professional nursing in accordance with and guided by the CHOP Nursing Professional Practice Model. The nurse at this level demonstrates competence in caring for patients and families and progresses to caring for those with the most complex problems/highest acuity in the designated clinical area. Job Description To maintain high-quality, medically necessary, evidence-based care, and efficient treatment of all patients, regardless of payment source, by ensuring the patients receive the right care, at the right time, in the right place. Case Management Model: utilize an Integrated Case Management Model. Under this model the Case Managers will follow patients through the continuum while facilitating the functions of utilization review, care coordination, advocacy and discharge planning. Collect, analyze, and communicate data that includes but not limited to quality of care, patient safety, patient/staff/physician satisfaction operational efficiency and financial performance. Collects Core Measure data concurrently. Track and trends avoidable day information in Midas per process. Performs other duties as assigned, including for example participation in planning sessions for departmental activities.
