Multidisciplinary rounds are done Monday – Friday. This involves the patient in their care and allows the team members to collaborate with the patient to give them the best possible care and appropriate discharge to home or other facilities. The Express Admit Unit (EXAU) is under the management of the CDU. There are 6-bed areas where the admitted patients stay until a floor bed is available. This gives them privacy and care while waiting for their room to be available. Both CDU and EXAU can have patients on telemetry for cardiac monitoring. The Nurse Navigator also performs follow-up calls within 24 hours of treatment, communicates findings with providers, coordinates ongoing care, and assists with management of the clinic In Basket, including patient messages, provider-assigned tasks, lab results, and other care coordination activities.
