Work closely with a multi-disciplinary team, including medical assistants, primary care providers, behavioral health providers, and others, to deliver holistic care. Facilitate patient access to internal programs such as the Health Outreach Program, Housing First, and Medication Assisted Treatment Program. Serve as a member of the Substance Use Disorder (SUD) treatment team, providing case management, crisis intervention, and individual counseling related to medication-assisted treatment. Oversee the care of patients with transitional, chronic, and complex health conditions, ensuring continuity of care and appropriate follow-up. Mentor and guide new nurses and nursing students, providing on-the-job training and support to build their clinical skills and confidence. Accurately document all patient care activities in the electronic health record (EHR), ensuring compliance with legal and organizational standards. Qualifications Summary Current licensure in the State of Colorado to practice as a Registered Nurse. Certification in Basic Life Support. Education & Experience A degree in nursing (ADN, BSN or MSN) from an accredited college or university is required. Experience in ambulatory, public, community, mental health, emergency, or critical care nursing is preferred. Pediatric nursing experience is highly desirable.
