What does the Clinic Registered Nurse (RN) do? Under general supervision, the Registered Nurse (RN) provides professional nursing care for patients adhering to national and company standards and guidelines for specialty regulations. Must recognize physical, psychological, and spiritual aspects of care and participates in company-wide quality initiatives. Supports and adheres to the US Oncology Compliance Program, to include the Code of Ethics and Business Standards. Responsibilities The essential duties and responsibilities: Plan, implement, coordinate, evaluate and promote the continuity of care consistent with established standards and practices. Assesses health status by interviewing patient regarding current status and medical history. Assesses patient/family needs and provides teaching and other instructional material. Provides psychosocial support to patients and families. Facilitates clinical operations and patient care Manages patient medication refill requests from the pharmacy telephone line and records the information in the patient chart. Facilitates and schedules consultations, pre-testing, diagnostic tests or procedures, referrals and future appointments for patients. Triages patient phone calls as needed and documents relevant information in the patient's chart. As necessary, facilitates communication between patient/family and interdisciplinary team and coordinates services related to patient care. Assist patients in locating available community resources. Inter professional collaboration Acts as a resource to LPN/LVNs and medical assistants. Qualifications The ideal candidate for the Clinic Registered Nurse (RN) will have the following background and experience: Graduate from an accredited program for professional nursing education, BSN preferred Minimum of 1 year of nursing experience Willing to consider an entry, mid, or senior level candidate Current RN state license with the State Board of Nursing in practicing state Current Basic Life Support (BLS) certification. Our patient population includes: CHF, COPD, PNA, ESRD, DM, AMS, Bariatric, Cirrhosis, Pancreatitis, acute strokes, and much more. Froedtert offers at least 9 weeks of orientation with a dedicated preceptor/s and positive mentoring in a hands-on learning environment. As an organization, you will find that we do not have mandatory over-time (even at the height of the pandemic). Skills that will be used in this unit include: Foley insertions, trach cares/suctioning, G-Tubes, wound cares, NG insertions, PIV/central line medications, PCA/CADD pumps and wound vacs. The Charge Nurse is a Registered Nurse with unit lead responsibilities for patient flow and adjustment of unit staffing to meet the dynamic needs of the patients. The Charge Nurse will use the nursing process to make ongoing evaluations of patients' acuity, assessments of the staff nurses workload, and anticipate fluctuations in admissions, discharges and transfers. The facilitation of patient flow includes triaging and prioritizing admissions and overseeing the care coordination of patients being discharged/transferred. Daily nurse staffing includes employing effective decision-making behaviors that are used to safely balance changes in patient acuity and census with the availability, experience and competency of the staff nurses, to ensure safe and adequate staffing for all shifts. Nurse Clinician (Nurse Navigator/Case Manager) - Emergency Department - 60%

The University of Iowa Health Care, Care Coordination Division is seeking a Nurse Clinician to function as a clinical nurse expert within the Emergency Department and care coordinator for the assigned patient population facilitating seamless transitions across the continuum of care. The nurse functions as an administrative and clinical liaison for individuals to optimize quality health care delivery using clinical assessment/screening tools, education reinforcement, follow-up on the plan of care recommendations and guiding the utilization of cost-effective resources. This position will manage a specified population and work closely with other care coordinators during transitions of care as well as other interdisciplinary team members across the Continuum of Care. Duties To Include Collaborate with nursing staff and other health care professionals in the development and review of standards and in planning, implementing and evaluating patient care for a specific patient population. Communicate with physicians and members of the health care team directing patient care. Provide direct nursing care, facilitate continuity of care and follow universal precautions. Coordinate and facilitate patient care between multiple disciplines including home health agencies, referring physicians and other community support services. Coordinate patient care to better utilize supportive services through patient visit and possible admission, pre-schedules testing, follows up with test results to both patient and physician, obtains medical history, coordinates follow up visit appointment, provides medication, rehabilitation teaching, etc. Serve as a clinical resource or consultant for specific patient population. Participate in development and implementation of managed care programs for a specific patient population. Provide leadership for direct patient care programs in area of specialization collaborating with unit-based nurse management team(s). Participate in the ongoing implementation of current regulatory standards in order to meet state and national agency accreditation. Design and implement patient/family health education activities for the patient population. Serve as an educational and clinical resource to patient/family/staff. Support the implementation and evaluation of staff education and orientation programs. Facilitate the educational experience for health care students and monitor the care delivered. Incorporate the role of patient care provider, advocate (patient, family, and health care professional), consultant, educator, researcher and quality manager.
