Ensure that exam rooms and/or triage area is stocked for necessary supplies and materials and that appropriate staff maintains a list of medical supplies. Effectively communicates and actively engages in interdisciplinary team meetings, family meetings, and clinic meetings. Participates within the interdisciplinary team in the formulation of Plans of Care participants, as well as in other interdisciplinary team settings. May be required to provide after hours and holiday coverage. Complies with all regulatory and policy and procedure guidelines. Duties and responsibilities may include and are not limited to: Collaborating with medical, mental health and correctional staff to enhance quality patient care while maintaining a safe and secure work environment Recruiting, hiring, training, supervising and evaluating nursing staff, ensuring competency and adherence to policies Effectively communicating with healthcare staff and correctional staff to coordinate patient care with providers outside of the facility Monitoring compliance with site-specific and company policies and procedures, taking appropriate corrective action, and ensuring compliance with ACA, NCCHC standards Manages the daily operations of the medical unit, including scheduling, budgeting, and record-keeping, ensuring adequate nursing coverage for all shifts Ensuring all heath care related equipment and supplies, including pharmaceuticals and over the counter medications, are appropriately stocked and available at all times Maintaining current and accurate patient records and documentation Developing staff, conducting in-service training for new and current staff Conducting daily clinical rounds to all designated areas of responsibility to observe and review documentation, orders, and patient care plans, and ensuring objectives are understood and practiced by the nursing staff Assisting in daily triage/sick call, minor medical/surgical procedures, and patient assessments Participating in all monthly, quarterly, and annual meetings as assigned and required in accordance with contract compliance Mitigating risks by proactively addressing potential hazards, including dangerous work conditions, health and safety concerns and reporting issues to company leadership and facility management Qualifications Experience / Requirements / Qualifications Holds a current, unrestricted license as a Registered Nurse within state of Florida. Must possess an active BLS CPR Certification (American Heart Association preferred) Progressive administrative/management experience in a healthcare setting preferred Strong clinical skills and progressive management experience providing comprehensive medical, dental, and mental healthcare services within a correctional healthcare or acute care setting Ability to manage complex staffing matrices and clinical supplies Excellent communication skills with proven experience managing and training medical staff Knowledge of nursing practice principles and techniques, occupational health and safety standards, infectious disease management, applicable health care laws, regulations, and procedures for direct health care services Ability to maintain confidentiality of all proprietary and/or confidential information Integrity, professionalism and ability to adhere to a Code of Conduct and comply with all facility(s) correctional healthcare policies, procedures, and legal requirements Must pass the facility’s criminal background check and drug screening Physical Capabilities: Must have the ability to stand for long periods, lift and move patients (up to 50 pounds), bend, squat, reach, push, pull, walk significant distances, and perform physically demanding tasks like CPR, all while maintaining good balance and endurance throughout a shift. This position is a 100% onsite opportunity. You will need to be based in the greater Chicagoland area. Under the direction of a faculty sponsor and in collaboration with the program support team, the Nurse Navigator will coordinate the care of patients who have recently undergone a heart transplant. The Nurse Navigator will act as a clinical liaison for patients, across medical services and clinical departments. They will assess clinical, emotional, spiritual, psychosocial, financial, and other patient needs; as well as evaluate and manage patient symptoms before, during, and after treatment. Essential Job Functions Provides care coordination & patient education to patients in the service line. Supports discharge planning for the service line. Overview The Registered Nurse (RN) provides nursing care to patients utilizing the nursing process and standards of practice for a professional nurse. The RN is expected to perform all staff nurse activities and function as charge nurse when assigned. The RN maintains overall accountability/responsibility for assigned patients, even though some activities/tasks may be delegated to Technical Nurses, Patient Care Associates, and/or Sitters. Required Education: Associate's Degree Specific course work: Graduate of an accredited school of nursing. New hires effective June 14, 2017, must have a BSN degree from an accredited school of nursing within three (3) years of hire or transfer into the position.
