At the time of a questionable admission, the Clinical Access Coordinator will have the knowledge to direct the physician to an alternate level of care including skilled care, nursing home or outpatient services. The Public Health Nurse Case Manager will be responsible to assist with development and coordination of services with a multi-disciplinary care team to positively impact services to high-risk groups to contain communicable disease outbreaks including foodborne illnesses, immunization preventable illnesses, TB, Sexually Transmitted Infections (STIs) and Coronavirus .
