Registered Nurse - Outpatient Hospice (Case Manager)

Full-time Outpatient Hospice opportunity. Prefer prior Hospice or Home Health experience $5,000 sign on Bonus with 2+ years related experience. Adult and pediatric patients are cared for as outpatients in their place of residence with access to a hospice nurse 24/7.We provide a care team that includes RNs, home health aides, social workers, chaplains and providers that provide personal care to patients and complimentary therapy with massage. Additionally we have comprehensive volunteer services and a bereavement program.We also have a pediatric hospice program and provide hospice services in the Benson area. Summary Coordinates patient care planning with other disciplines utilizing the principles of the nursing process and critical thinking skills in order to maintain patient safety and well-being. Essential Functions Provides direct patient care, evaluates outcomes, consults with other specialists as required and adjusts nursing care processes as indicated to ensure safe patient care. Writes or obtains initial nursing histories, assesses patients' conditions and develops individual care plans; maintains patient documentation. Receives and reviews preceding shift reports and floor nurse reports; makes complete rounds of all units, and notes patients' condition. Notifies appropriate physician regarding any unusual or unexpected events or problems requiring direct physician consultation. Works with other members of the interdisciplinary patient care team, implements and reinforces patient instructional needs with patient and family offering informational material and pointing out special areas of concern. Evaluates outcomes of patient care, consults with other specialists as needed and adjusts nursing care processes as necessary to ensure optimal patient care. Identifies and resolves workplace health and safety issues. P rovides appropriate, direct patient care utilizing appropriate aseptic techniques. May manage IV lines, ensure that sterile techniques are adhered to, and oversee patient movement and positioning to ensure that the patient is handled in a medically safe manner. May perform the administration of pharmacological vasodilator medications for the purpose of nuclear myocardial perfusion imaging; complies with examination competency as defined in the Imaging Services Practice Standards. Complies with policies, procedures, regulations, and standards and takes corrective action when violations occur. Orients, instructs, and educates assigned personnel. Adheres to and supports team members in exhibiting TMCH values of integrity, community, compassion, and dedication. Adheres to TMCH organizational and department-specific safety, confidentiality, values, policies and standards. Performs related duties as assigned. Minimum Qualifications EDUCATION : Bachelor’s degree preferred. EXPERIENCE : One (1) year of nursing experience in an acute care setting preferred. (Positions assigned to EP: require two (2) years of EP experience or demonstrated competency in all areas of EP Lab procedures). LICENSURE OR CERTIFICATION: Current RN licensure permitting work in the State of Arizona and Basic Life Support (BLS) certification required. Additional certification may be required per department. Knowledge, Skills, And Abilities Knowledge of direct patient care and critical care procedures and techniques, tools, and responses required to ensure optimal patient care. Knowledge of exam indicators, contraindications, and patient preparation. Skill in communicating in a clear and concise manner with staff involved in critical care, and physicians to ensure the proper care of patients. Skill in reading 12-lead EKG. Skill demonstrated in assigned disciplines such as EP, directly involved in patient care. Ability to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals. Ability to complete routine reports and correspondence. Ability to listen and accurately interpret others’ communication or instructions to take appropriate action. Ability to speak and communicate effectively. Ability to define problems, collect data, establish facts, and draw valid conclusions. Ability to interpret an extensive variety of instructions and deal with several abstract and concrete variables.

Reports must be submitted immediately when the reporting person has reasonable cause to believe that abandonment, abuse, financial exploitation, or neglect of a vulnerable adult has occurred. 6. Teaches the patient and family/caregiver self-care techniques as appropriate. Provides medication, diet and other instructions as ordered by the physician and recognizes and utilizes opportunities for health counseling with patients and families/caregivers. Works in concert with the interdisciplinary group. 7. Provides and maintains a safe environment for the patient. 8. Assists the patient and family/caregiver and other team members in providing continuity of care. 9. Assesses and evaluates patient’s status by: A. Writing and initiating plan of care B. Regularly re-evaluating patient and family/caregiver needs C. Participating in revising the plan of care as necessary 4. Initiates the plan of care and makes necessary revisions as patient status and needs change. 5. Uses health assessment data to determine nursing diagnosis. 6. Develops a care plan that establishes goals, based on nursing diagnosis and incorporates palliative nursing actions. Includes the patient and the family in the planning process. 7. Initiates appropriate preventive and rehabilitative nursing procedures. Administers medications and treatments as prescribed by the physician in the physician’s plan of care. 8. Counsels the patient and family in meeting nursing and related needs. 9. Provides health care instructions to the patient as appropriate per assessment and plan. 10. Assists the patient with the activities of daily living and facilitates the patient’s efforts toward self-sufficiency and optional comfort care. 11. Acts as Case Manager when assigned by Clinical Supervisor and assumes responsibility to coordinate patient care for assigned caseload. Communication 1. Completes, maintains and submits accurate and relevant clinical notes regarding patient’s condition and care given. Records pain/symptom management changes/outcomes as appropriate. 2. Communicates with the physician regarding the patient’s needs and reports changes in the patient’s condition; obtains/receives physicians’ orders as required. 3. Communicates with community health related persons to coordinate the care plan. 4. As a mandatory reporter, reports failure to comply with the requirements of the Dept. of Health as required within 14 calendar days, using Dept. of Health forms. 5. Reports suspected abandonment, abuse, financial exploitation, or neglect of a person in violation of the department of social and health services and the proper law enforcement agency. Reports must be submitted immediately when the reporting person has reasonable cause to believe that abandonment, abuse, financial exploitation, or neglect of a vulnerable adult has occurred. 6. Teaches the patient and family/caregiver self-care techniques as appropriate. Provides medication, diet and other instructions as ordered by the physician and recognizes and utilizes opportunities for health counseling with patients and families/caregivers. Works in concert with the interdisciplinary group. 7. Provides and maintains a safe environment for the patient. 8. Assists the patient and family/caregiver and other team members in providing continuity of care. 9. Works in cooperation with the family/caregiver and hospice interdisciplinary group to meet the emotional needs of the patient and family/caregiver. 10. Attends interdisciplinary group meetings. Additional Duties 1. Participates in on-call duties as defined by the on-call policy. 2. Ensures that arrangements for equipment and other necessary items and services are available. 3. Supervises ancillary personnel and delegates responsibilities when required. 4. Assumes responsibility for personal growth and development and maintains and upgrades professional knowledge and practice skills through attendance and participation in continuing education and inservice classes. 5. Fulfills the obligation of requested and/or accepted case assignments. 6. Actively participates in quality assessment performance improvement teams and activities. POSITION QUALIFICATIONS 1. Graduate of an accredited school of nursing 2. Registered Nurse, with a License in good standing to practice. 3. Registered nurses shall have a minimum of (a) one (1) year of experience as a professional nurse within the last three (3) years; OR have a baccalaureate degree in nursing and minimum of two (2) years’ experience, at least one of which is in the area of public health, home care, or hospice nursing is preferred. 4. Management experience not required. Responsible for supervising hospice aides. 5. Once an offer of employment is made, it is contingent upon satisfactory references, as requested, and criminal background checks by regulation. 6. Excellent observation, verbal and written communication skills, problem solving skills, basic math skills; nursing skills per competency checklist. 7. Prolonged or considerable walking or standing. North and South 8 have telemetry capabilities for our patients. Kelemen 8 has frequent admissions and discharges. Kelemen 8 can accept a total of 77 patients. The North side houses 37 patients, and the South side houses 36 patients. There are two negative pressure rooms with an anti-room. There is a pharmacist present on the unit to assist the RNs with medication questions/concerns. Multidisciplinary rounds occur every Monday through Friday. Rounds involve all aspects of the healthcare team, including the patient and family.
