Neurology - Nurse Practitioner - Center City

Job Details The Neurology Nurse Practitioner (NP) provides comprehensive clinical support to faculty within the Jefferson Cognitive Neurology Division, focusing on the evaluation, diagnosis, and management of outpatient populations with cognitive neurological disorders. This role works collaboratively with attending physicians and interdisciplinary team members to deliver high-quality, patient-centered care, while practicing within the legal scope of licensure. The NP plays a key role in the care of patients with dementia, including both neurological and behavioral aspects, as well as ongoing care management and coordination with caregivers and support services. Responsibilities include conducting patient assessments, developing and implementing treatment plans, monitoring disease progression, and providing education to patients and families. This position also supports emerging treatments and management strategies for patients with Mild Cognitive Impairment (MCI), including assisting with the evaluation of eligibility for new therapies and monitoring treatment response. In addition, the NP provides consultation services for neurology patients presenting with psychiatric or behavioral syndromes, functioning as part of a specialized “memory clinic” model of care. This includes collaboration with psychiatry and other specialties as needed to ensure comprehensive management of complex cases. The Neurology NP contributes to maintaining clinic efficiency, enhancing patient outcomes, and supporting the academic and clinical mission of the division through excellence in care delivery and teamwork. Job Description Assessment & Management Performs complete history & physicals and documents in the medical record. Orders and collects data using appropriate assessment techniques, relevant supporting diagnostic information and diagnostic procedures where indicated. The APP may serve as the first responder for both nursing and house staff in the event of patient status changes, orders, and in situations requiring procedural and resuscitative interventions. Assesses for risks associated with the care of the acute and complex chronically ill patient including: medication side effects, immobility, impaired nutrition, fluid and electrolyte imbalance, immunocompetence, invasive interventions and diagnostic procedures. Diagnosis Collaborates with interdisciplinary health care team in making diagnoses of acute and complex chronic conditions. Manages diagnostic tests through ordering, interpretation, performance, and supervision. Formulates differential diagnoses by priority. Diagnoses complications and orders appropriate interventions Formulates Plan of Care Identifies expected outcomes from diagnosis, and formulates and documents a plan of care to address complex acute and chronic health care needs. Utilizes evidenced based practice guidelines and protocols in an individualized, dynamic plan of care that can be applied across the continuum. Implements and modifies plan of care. Prescribes diagnostic strategies and therapeutic interventions. May perform advanced procedures consistent with privileges and competency validation. Documentation Documents/dictates key components of patient's progress via daily progress note, transfer, and discharge summary, and or clinic note where applicable. (h&p, daily progress notes, plan of care, problem lists, procedure notes, acute event note, discharge summaries, in medical record per specific patient unit or service, death summary) Documentation is timely, meets acute care compliance standards and captures patient acuity. Communication and Collaboration Maintains ongoing communication and collaboration with interdisciplinary health care team. Facilitates and communicates with patient, family and staff to promote continuity of care across the inpatient and outpatient continuum of care. Professional Practice Demonstrates Professional Practice behaviors including: preceptor/mentoring, education and instruction of graduate students. These protocols will be determined by area of specialty. 3. Practice Agreements Practice agreements shall include provisions for the following: a. Patient Assignment Patients are assigned to the Nurse Practitioner or physician according to the needs of the patient. b. Diagnostic Tests Laboratory tests may be ordered according to the initial work-up guidelines found in the reference protocol. Collaboration is not necessary for routine diagnostic procedures. However, consultation and documentation should be completed before more extensive tests are ordered, e.g., nuclear medicine tests, MRI, CT scan, IVP, etc. c. Consultation Nurse Practitioners consult with and refer patients to subspecialty clinics as needed. d. Prescriptive Practice Nurse Practitioners prescribe medications according to the New York State Laws and Regulations for prescription writing and record-keeping (see Appendix). e. Charting Nurse Practitioners are responsible for documenting findings, consultations and plans of care in the patients' medical records. .
