About the Role We are currently looking for an experienced Medical Assistant to join our team. The Licensed Practical Nurse (LPN) works under the direction of the Team Leader. He/she gathers assessment data, monitors, follows plan of care and promotes optimal level of health. The LPN provides medication administration, treatments, supervision of CENAs performing ADLs. Collaborates with the Care Coordinators and other team members to ensure communication and timely updates on residents change in conditions. We do 12 hours shifts from 6:30am-7pm and 6:30pm-7am. Responsibilities KEY RESPONSIBILITIES INCLUDE: • Gathers assessment data, monitors comprehensive physiological, psychological, sociocultural, and spiritual needs of patients/significant others under the supervision of the Team Leader.
