Job Summary Provides administrative, operational, and clinical responsibilities for all services in the facility on evenings, nights, weekends, and/or holidays. Serves as the clinical resource person on patient care and services; acts as the patient care representative; collaborates with medical staff, ancillary support, and clinical services in order to ensure optimal patient care services and environmental conditions at all times. Responsible for notifying Administrator-On-Call of serious issues, incidents, and internal and external disasters. Essential Functions Supervise patient care through verification of the use of the nursing process according to established nursing practices and patient care standards and provide direction and guidance to facilitate the nursing process. -Ensure appropriate intervention is demonstrated in all situations, e.g., deterioration of patient condition, disruptive behavior, equipment malfunctions, etc. -Serve as off-shift patient care representative, listen to patient or patient family members' issues, documents, and complaints, and investigate if required. -Conduct rounds of all patient care areas and define expectations for accurate reporting with emphasis on high-acuity patients. -Collaborate throughout off-shift with support services, emergency, admitting, and clinical services to coordinate clinical patient care or environmental needs. -Follow through to ensure patient's needs, as well as environmental and safety-related issues, are resolved. BAYADA Home Health Care has an immediate opening for a Registered Nurse Executive to fill the role of Division Director of Clinical Operations in our Assistive Care (AC) and Assistive Care State Programs (ACSP) practices. This position requires an RN with senior level experience in clinical program development, care service delivery, and outcomes focused clinical excellence. Position Summary: HSL provides enhanced housing with services to seniors living in its four affordable housing locations and at partner housing sites, intending to improve quality of life and support independent living. At its core, enhanced housing with services is a proactive approach wherein resident services staff regularly and actively reach out to each individual resident to engage with them around their health and wellness, identify areas of need/risk, and provide intensive, individualized case management and support as needed and desired by the resident. This role is spread across five Brookline Housing Authority senior sites in Coolidge Corner. The Nurse Care Manager is a key member of the housing team working to support residents in living independently and safely for as long as possible by developing meaningful relationships with residents and providing support in a holistic way. The Nurse Care Manager collaborates with team members to engage residents in wellness assessments and health education, connect them to needed services, support in managing health concerns, coordinate care for residents returning from hospital or rehabilitation stays, document all resident care tasks, and partner with community providers to promote overall well-being. Provides thorough and concise documentation of patient’s histories, assessments, plans, and treatments. Enters messages as staff messages or telephone encounters in the computer. Enters and views patient letters in the computer. Enters exam encounters in the computer. Consistently participates in the activities in the transition of care, recalls, high risk management, in basket management, and vaccines. Follows protocols for management of patients with chronic/acute conditions and transitions of care. Accepts constructive feedback and incorporates into plan for professional growth and development. Recognizes and meets the safety needs of the departmental patient population. Participates in the orientation, training, and mentoring of new employees. Provides input into the development of the orientation plan for new employees. Actively participates in NeighborHealth programs, committees and departmental meetings. SKILLS/ABILITIES: Demonstrated ability to communicate effectively both in writing and verbally.
