Job Description The Nurse Coordinator plays a vital role in the Lung Transplant Program(s), offering direct care to patients with complex medical conditions. They serve as the primary point of contact, coordinating pre-transplant evaluations, managing post-transplant follow-up care, and providing patient education. This position requires collaboration with a diverse team, including physicians and support staff, to deliver exceptional care and manage the unique needs of transplant patients. Responsibilities % of time Essential Function (Yes/No ) Key Responsibilities (To be completed by Supervisor) PRE-TRANSPLANT ASSIGNMENT – Scheduling of patient evaluations, patient education, and orientation to the transplant process.Monitoring of the evaluation process for patients, ensuring that the patient is being moved through the evaluation process in a timely manner.Acts as a liaison for multiple hospital departments during the patient evaluation process.Facilitate patient activities with consulting departments and physicians. Qualifications Required qualifications: Minimum of five (5) years of adult solid organ transplant nursing experience as an outpatient or inpatient Transplant Coordinator. Five (5) years of RN experience in end-stage organ-specific disease, transplantation, and managing patients with organ failure. Proven organizational skills and the ability to work independently, managing multiple concurrent tasks in a high-stress environment. Excellent communication skills, both written and verbal, and the ability to conduct oneself professionally. Computer literacy, including proficiency in MS Word, Outlook, internet use, and a willingness to learn and adapt to new EHR systems. Strong MS Office skills, particularly in Word, Excel, and PowerPoint, and proficiency in using databases. Active Registered Nurse (RN) licensure in the State of California, ensuring compliance with regulatory standards. Active American Heart Association BLS certification, demonstrating commitment to patient safety and quality care. Preferred qualifications include a BSN or MSN degree, as well as experience in assessing advanced heart failure/cardiac and/or pulmonary disease.
