This involves the patient in their care and allows the team members to collaborate with the patient to give them the best possible care and appropriate discharge to home or other facilities. There is a pharmacist present on the unit to assist the RNs with medication questions/concerns. Multidisciplinary rounds occur every Monday through Friday. Rounds involve all aspects of the healthcare team, including the patient and family. About the Role: As an LPN at Care Pavilion, you will be responsible for providing high-quality patient care while working collaboratively with our interdisciplinary team of healthcare professionals.
