The Family Connects model is an evidence-based and successfully demonstrated program that connects parents of newborns to the community resources they need through postpartum nurse home visits. The model was developed in 2008 through a partnership with Duke University, the Center for Child & Family Health (CCFH), and the Durham County Health Department. As a community-based program, Family Connects supports new parents in caring for their newborn(s), offers physical assessments of the mother and the baby, addresses questions about caring for the newborn(s), identifies parents’ needs, and helps to identify community services or resources that can meet those needs. In addition, through this connection to resources, Family Connects helps to identify gaps in critical community-wide resources with the goal of working toward increasing needed services locally. A Family Connects Home Visiting Nurse provides home visiting to all families with newborns in Chicago. The goal is to support and assess family needs in 12 domains of physical and psychosocial well-being and facilitate referrals and connections to community agencies to support families in these areas. The Home Visiting Nurse will work in a team environment of clinical and non-clinical staff in order to schedule visits, and support families in the community using the evidence-based Family Connects home visiting protocol. In addition to home visiting responsibilities, the Home Visiting Nurse will support the Prenatal Education Department by teaching a minimum of one in-person prenatal education class per week, as assigned. This responsibility includes approximately two hours of classroom instruction and one hour of preparation time per class. The nurse will deliver evidence-based education on topics such as pregnancy, labor and birth, postpartum recovery, newborn care, breastfeeding, and other maternal-child health topics, while serving as a clinical resource for expectant and new families. Other Information Required Job Qualifications: High school diploma/GED required. Clinical Medical Assistant Certification by AAMA, AMT, NCCT, NAHP, AAH, or NHA required (includes principles and techniques of injections and medication administration) required upon hire. Graduates of a Rush Medical Assistant Apprenticeship program are required to obtain Clinical Medical Assistant Certification by AAMA, AMT, NCCT, NAHP, AAH, or NHA required within six months of hire. Preferred Job Qualifications One year clinical experience. Phlebotomy experience. Epic Ambulatory experience. Responsibilities Coordinates and maintains organized patient flow. Performs patient intake: vital signs, height and weight, reason for visit, interim history, allergies, medication lists, and other interventions as required. Prepares patient for exam based on reason for visit. Prepares and supports patient and assists provider with procedures. Reviews appointment schedules and ensures all patient charts are available for the visit. Reviews patient charts and ensures that all ancillary reports and required paperwork are available to the provider at the time of visit. Performs basic lab and testing procedures according to standards. Performs phlebotomy and EKGs. Administers immunizations as ordered by Physician. Administers RUMG approved intramuscular or subcutaneous medications after competency has been assessed, verified and documented. Performs nebulizer treatments in the clinic setting after competency has been assessed, verified and documented. Performs Tuberculin skin testing upon the order of the Physician. Demonstrates appropriate population specific techniques with patients. Documents administration of over-the-counter medications and/or other medications in Epic per policy or protocol. Cleans, prepares and stocks exam rooms, treatment rooms, and workstations. Calls patients regarding test results and medical instruction under the direction of a Physician. Calls in medication refills under the direction of a Physician or protocol. Documents patient care information in the outpatient medical record according to standards. Assists with scheduling of patient tests, procedures, and follow-up appointments. Assists in the care, cleaning, and sterilization of equipment and instruments. Maintains the inventory of medical and linen supplies. Maintains quality control for equipment. Provides cross coverage to other clinical areas as assigned. Participates in quality improvement projects. Maintains compliance with The Joint Commission (TJC) standards and other regulatory bodies. May have Unit specific duties as needed.
