JOB DESCRIPTION: SUMMARY OF POSITION The Psychiatric-Mental Health RN (PMH-RN) in Supportive Housing Services is a member of the interdisciplinary team which partners with individuals to achieve independence in their physical and mental health care needs. Provide health promotion, screening, maintenance, and education based on the individual’s integrated health care needs. Assessment and monitoring of health care, case coordination, and self-care needs are part of the ongoing nursing process. Administration and oversight of treatment regimens as ordered by the prescribing provider. Support of non-clinical staff related to patient’s medical/clinical needs. Management of all patient/client care needs related to medication, point of care lab, and infection control. The PMH-RN in Outpatient Psychiatry prepares patients for the clinic appointment with the psychiatric physician or nurse practitioner, obtaining history and vital signs, ensuring ordered labs/procedure results are received, providing psychoeducation, and administration of prescribed injectable medications. This RN will also ensure timely telephone triage responses, lab result notifications, and medication prior authorization processes.
