Paradigm is looking for a full-time, 0.75 - 1.0 FTE (30-40 hours per week), benefited Field Nurse Case Manager to cover the Saginaw and Bay City, MI, areas . This is a salaried position: base salary range at 0.75 FTE is $68,000-$73,000. At 1.0 FTE, base salary range is from $88,000-$93,000 annually. A Field Case Manager role is a home-based position with travel, up to 2 hours one-way (4 hours round trip), to doctors’ offices, hospitals, and various other locations. This individual is responsible for the medical case management of work-related injuries, which includes assessment, planning, coordination, implementation, and evaluation of injured/disabled individuals. The Case Manager works with insurance carriers, medical care providers, attorneys, employers, and employees, and closely monitors the progress of the injured worker and reports results back to the employer and insurance carriers. Position Summary: Responsible for supervising McLaren Bay Region staff in performing patient related clinical activities in area of jurisdiction for assigned shift. Responsible for ensuring patient placement is timely and appropriate, based on patient need and available resources. Essential Functions and Responsibilities : Uses independent judgement to exercise authority on behalf of the Medical Center, including: Disciplining subordinates according to Medical Center procedure. Assigning work to employees and directing the workflow in area of jurisdiction Resolving employee complaints/concerns Conducting periodic performance appraisal interviews with subordinates if requested/assigned. Responsible for ensuring patient placement is timely and appropriate, based on patient need and available resources. Coordinated patient placement with input from Admitting, Managers, and Physicians. Assesses, prioritizes, and allocates the resources necessary to address the needs of patients, based upon defined standards of care and pre-established staffing matrixes. Responsible for timely concise and professional communication. Assists managers/directors in relaying pertinent information to staff and reinforcing changes in policies and procedures. Provides feedback to Patient Care Services Administration regarding concerns, suggestions, and/or significant events. Responsible for supervising McLaren Bay Region staff in performing patient related clinical activities in area of jurisdiction for assigned shift. Evaluates the performance of orientees on assigned shifts. Responsible for assisting with patient crisis situations including various in-house codes. Qualifications: Required: Bachelor’s Degree in Nursing, or Associate’s Degree in Nursing with three (3) years of nursing experience Valid Michigan Registered Nurse License BLS issued by American Heart Association or Red Cross Two Years clinical experience Preferred: Bachelor’s degree in nursing.
