Job Summary: The Registered Nurse (RN) provides holistic patient care service as a member of the primary care team. The RN performs routine clinical tasks necessary for efficient patient care and assists other patient care providers as needed. This position is an On Call position. Essential Duties and Responsibilities: Assess patient needs through problem-oriented history, including past medical history, current problems and vital signs. Identify patients with significant health risks, including patients at risk due to hereditary factors, lifestyle, diet, substance abuse, social/cultural/environmental factors, and/or inadequate understanding of their disease process or treatment plan. Perform patient education involving explanation to the patient of diagnosis, treatment plan (including use of medications and their effects and side effects), infection control (if indicated), and preventative techniques to lower patient risk of future disease or accident. Assess patient needs through telephone interviews · advise patients over the phone and identify patients with urgent care needs. Triage patients who arrive without appointments, including assessment and evaluation for need of medical care based on their medical history, present complaint, vital signs, and physical assessment (after physician consult if necessary). Dispense medication under M.D. supervision and administration of oral, I.M. and I.V. medication as ordered by M.D., FNP, and CNW. Case manage high-risk patients. Facilitate patient/family group education sessions. Coordinate patient flow and triage patients in the absence of the provider when provider is out or unavailable. Update on-call providers daily on changing needs of complex, high maintenance or high-risk patients. Maintain infection control within the clinic. Perform laboratory procedures when necessary. · Handle protected health information (PHI) in a manner consistent with the Health Insurance Portability and Accountability Act of 1996 (HIPAA). · Valid driver's license, reliable transportation, safe driving record and insurance coverage required. · Perform other duties as assigned. HIPAA Requirements: The RN will use PHI to answer patient questions about their medical problems or plan of care, to triage and schedule appointments for urgent medical problems, to relay information from the health care providers to the patient and to provider nursing visits at patients homes. Applying the minimum necessary rules of HIPAA, the designated record sets to which this employee will have access include: scheduling, demographics, and view only patient account information in the practice management system, and the complete medical record, including confidential sections. General Purpose The primary purpose of your job position is to provide each of your assigned residents with routine daily nursing care and services in accordance with the resident’s assessment and care plan, and as may be directed by your supervisors. Essential Duties • Use the wristband or photo card file to identify residents before administering treatments, serving meals, etc., as necessary. • Use only authorized abbreviations established by this facility when recording information. • Report all changes in the resident’s condition to the Nurse Supervisor/Charge Nurse as soon as practical. • Record all entries on flow sheets, notes, charts, etc., in an informative, descriptive manner. • Report all accidents and incidents you observe on the shift that they occur. • Agree not to disclose resident’s protected health information and promptly report suspected or known violations of such disclosure to the Administrator. • Report any known or suspected unauthorized attempt to access facility’s information system. • Perform only those nursing care procedures that you have been trained to do. • Ensure that the resident’s room is ready for receiving the resident (i.e., bed made, name tags up, admission kit available, etc.). • Greet residents and escort them to their room. • Introduce resident to his/her roommate, if any, and other residents and personnel as appropriate. • Make resident comfortable. • Inventory and mark the resident’s personal possessions as instructed. • Store resident’s clothing. • Assist residents with packing their personal possessions when they are being transferred to a new room, or when being discharged. • Transport residents to new rooms or to the receiving area. • Assist with loading/unloading residents’ to/from vehicles as necessary. • Create and maintain an atmosphere of warmth, personal interest and positive emphasis, as well as a calm environment throughout the unit and shift. • Meet with your shift’s nursing personnel, on a regularly scheduled basis, to assist in identifying and correcting problem areas, and/or the improvement of services. • Report all complaints and grievances made by the resident. • Participate in and receive the nursing report as instructed. • Follow established policies concerning exposure to blood/body fluids. • Make beds as instructed. • Put extra covers on beds as requested. • Measure and record temperatures, pulses, and respirations (TPRs) of residents, as instructed. Weigh and measure residents as instructed. • Ensure that residents who are unable to call for help are checked frequently. • Answer resident calls promptly. • Check residents routinely to ensure that their personal care needs are being met. • Assist residents with identifying food arrangements (i.e., informing resident with sight problem of foods that are on his/her tray, where it is located, if it is hot/cold, etc.). • Record the resident’s food/fluid intake. Report changes in the resident’s eating habits to the Nurse Supervisor/Charge Nurse. • Keep residents’ water pitchers clean and filled with fresh water (on each shift), and within easy reach of the resident. • Perform after meal care (i.e., remove trays, clean resident’s hands, face, clothing, etc.). • Participate in and receive the nursing report upon reporting for duty. • Perform all assigned tasks in accordance with our established policies and procedures, and as instructed by your supervisors. • Follow work assignments, and/or work schedules in completing and performing your assigned tasks. • Cooperate with inter-departmental personnel, as well as other facility personnel to ensure that nursing services can be adequately maintained to meet the needs of the residents. • Notify the facility when you will be late or absent from work. • Report occupational exposures to blood, body fluids, infectious materials, and hazardous chemicals in accordance with the facility’s policies and procedures governing accidents and incidents. • Assist residents with daily functions (dental and mouth care, bath functions, combing of hair, dressing and undressing as necessary). • Keep residents dry (change gown, clothing and linens, when it becomes wet or soiled). • Assist residents in preparing for medical tests (i.e., lab work, x-ray, therapy, dental, etc.). • Assist with lifting, turning, moving, positioning, and transporting residents into and out of beds, chairs, bathtubs, wheelchairs, lifts, etc. • Perform restorative and rehabilitative procedures as instructed. • Assist in preparing the resident for a physical examination. • Check each resident routinely to ensure that his/her personal care needs are being met in accordance with his/her wishes. • Observe and report the presence of pressure areas and skin breakdowns to prevent decubitus ulcers (bedsores). • Report injuries of an unknown source, including skin tears. • Observe disoriented and comatose residents. Record and report data as instructed • Perform special treatments as instructed. • Immediately notify the Nurse Supervisor/Charge Nurse of any resident leaving/missing from the facility. • Use only the equipment you have been trained to use. • Report defective equipment to the Nurse Supervisor/Charge Nurse • Maintain the confidentiality of all resident care information including protected health information. Our nurses work collaboratively with physicians, serving as a key liaison between the physician and the patient. This role offers the opportunity to travel to various hospitals and Red Cross sites. Responsibilities include performing specialized procedures such as plasma exchange, red cell exchange, leukapheresis, stem cell harvests, photopheresis, therapeutic phlebotomies, platelet reductions, and white blood cell depletion. WHERE YOUR CAREER IS A FORCE FOR GOOD (Key Responsibilities): You will perform Apheresis procedures autonomously in multiple settings, including critical care, emergency room, and freestanding outpatient centers. Perform venipunctures, access central venous catheters, and access vortex and bard ports. Perform all phases of therapeutic apheresis procedures (plasma exchange, red cell exchange, platelet, and white cell reduction, photopheresis, and hematopoietic progenitor cell collection), to ensure safe and appropriate care of the patient and explain, as needed, the procedures for patients and their family and other attending healthcare professionals. Act as a consultant to external health care personnel about clinical apheresis. Assess patient status relative to therapeutic apheresis, prior to and during procedures, utilizing the nursing process, laboratory testing results, and other clinically significant procedures. Coordinate closely with physicians and nurses at a variety of hospitals to arrange therapeutic apheresis treatments and facilitate coordinated care. Evaluate patient medication for compatibility and safety of apheresis procedure and determine when to contact the physician. Assemble, restock, QC, and maintain supplies and equipment in a state of readiness to maintain a clean, safe, and efficient work environment. The American Red Cross will provide all necessary training. No experience is required. Standard Schedule : Variable 1st shift Monday to Friday- 40 hrs per week - Rotating on calls WHAT YOU NEED TO SUCCEED (Minimum Qualifications): Current state licensure in WA and Oregon as a Registered Nurse, or the ability to obtain state licensure, is a must. Current CPR certification is required Bachelor’s degree OR a combination of education and work experience. Two years of experience in medical/surgical or critical care nursing, OR-related experience. Good vein selection and venipuncture skills are essential. Good verbal communication skills and the ability to effectively discuss procedures and medical issues with patients, families, and physicians. Customer service experience, effective verbal communication, and public relations skills are preferred. A current, valid driver's license and a good driving record are required.
