Nurse Practitioner - Cardiac Surgery - Center City, Philadelphia, PA

Job Details The Acute Care Nurse Practitioner within the ambulatory setting works in collaboration with, and is an integral member of, a multidisciplinary health care team. The APP provides comprehensive care at the cardiac surgery outpatient practice in Center City. Job Description Job/Practice Description Busy outpatient practice providing care to adult and geriatric populations Four-day work week (Monday–Friday) totaling 40 hours No weekend or holiday coverage required Position requires Adult Gerontological Acute Care Nurse Practitioner certification Assessment & Management Performs complete histories and physicals and documents in the medical record. Orders and collects data using appropriate assessment techniques, diagnostic information, and procedures where indicated. Serves as the first responder for patient status changes and situations requiring procedural or resuscitative interventions. Assesses risks associated with care of acute and complex chronically ill patients including medication side effects, immobility, nutrition, electrolyte imbalance, immunocompetence, and invasive interventions. Diagnosis Collaborates with the interdisciplinary health care team in diagnosing acute and chronic conditions. Manages diagnostic tests through ordering, interpretation, performance, and supervision. Formulates differential diagnoses by priority and identifies complications, implementing appropriate interventions. Plan of Care Identifies expected outcomes and formulates individualized, evidence-based plans of care. Prescribes diagnostic strategies and therapeutic interventions. Implements, monitors, and modifies plans of care as needed. May perform advanced procedures consistent with privileges and competency validation. Documentation Documents all aspects of care, including histories, progress notes, procedures, and discharge summaries, in compliance with standards and timeliness requirements. Ensures documentation captures patient acuity and meets compliance standards. Communication & Collaboration Maintains open communication and collaboration with the interdisciplinary care team. Communicates effectively with patients, families, and staff to ensure continuity of care across the continuum. Professional Practice Demonstrates professional practice behaviors, including precepting, mentoring, and teaching. Engages in research and evidence-based practice initiatives. Participates in Advanced Practice Grand Rounds and specialty meetings. Maintains continuing education units and professional organization memberships.
