Completes appropriate documentation of all evaluations, care, and services rendered in the electronic medical record to accurately reflect services in the patient record and to ensure appropriateness of billing. Communicates with referring or primary care physicians regarding patient care and treatment. Responds to telephone inquiries from clinic patients and provides appropriate medical care and/or counseling. Assists individuals to assume responsibility for the prevention of illness and the promotion of health maintenance and/or restoration of health. Observes the Munson Healthcare Advance Practice Provider prescriptive authority guidelines. Maintains knowledge of current medical trends and ongoing professional development in pertinent medical fields. Participates in and / or serves as a resource for medical, nursing, and community education programs. Meets department productivity requirements. Recognizes and understands the need for full daily schedules. Independently monitors own daily appointment schedule and works in conjunction with other staff members to fill open appointment times. JOB DESCRIPTION Job Summary Leads and supervises team of nurse practitioners responsible for primary, urgent, and preventative care services in various non-clinical community settings including homes, nursing facilities, shelters, pop-up clinics, or any environment where members feel comfortable and are receptive to care. Contributes to overarching strategy to provide quality and cost-effective member care. Essential Job Duties • Oversees team of nurse practitioners responsible for provision of evidence-based, high-quality patient care via in-person and telehealth visits. • Provides real time training and mentoring to practitioners by rounding regularly and directly engaging in patient care. • Provides education on clinical guidelines, coding, quality measures, and documentation excellence. • Ensures patient-focused and culturally sensitive medical care is delivered to patients, and access to care is available to all members. • Continuously optimizes clinical performance and achieves department and clinical goals including desired clinical outcomes, clinical documentation excellence, and cost-effective care. • Performs and coordinates alternating on-call coverage to triage any urgent lab results or clinical needs and develop appropriate plan of care. • Participates in the implementation of new clinical projects. • Provides direct medical care to all levels of patients and population groups, for example, adult, geriatric, pediatric and women’s health; using evidence-based guidelines, properly diagnoses and creates patient-focused plan of care and treatment. • Orders/performs pertinent diagnostic laboratory and radiology testing, prescribes medication and durable medical equipment, and places referrals as clinically appropriate. • Documents thoroughly and accurately in electronic medical records with a focus on ensuring continuity of care while complying with Health Insurance Portability and Accountability Act (HIPAA). • Addresses chronic and acute primary care complaints, ascertains medical urgency, and takes appropriate action based on evidence-based guidelines. • Seeks specialty consultation as needed. • Participates in community-based initiatives geared towards building relationships with the community while addressing gaps in health care, such as pop-up clinics and shelters. • Actively participates in and leads regional meetings hosted by managers, peers, senior leaders team and other department leads including workshops, townhalls, huddles, new programs, new pilots, and initiatives. • Facilitates ride along member visits for new practitioners to ensure appropriate training and support. • Hires, trains, develops and manages team demonstrates accountability for team performance and achievement of department-specific goals. • Drives up to 120 miles a day on a regular basis to a wide variety of locations within the assigned region(s) with potential for additional mileage on extended mileage days or special project days. • Carries up to 50 lbs. of equipment daily. Equipment can be transported using a backpack or a rolling bag, depending on preference. Required Qualifications • At least 5 years of experience as a nurse practitioner, or equivalent combination of relevant education and experience. • Active and unrestricted national certification from one of the following organizations: American Academy of Nurse Practitioners (AANP) or American Nurses Credentialing Center (ANCC). • Current state-issued license to practice as a Family Nurse Practitioner (FNP). • Current Basic Life Support (BLS) certification. • Valid and unrestricted driver's license, reliable transportation, and adequate auto insurance for job related travel requirements. Summary The Division of Gastroenterology, Hepatology and Nutrition (GHN) at Texas Children's Hospital (TCH) is developing a team of close-knit Baylor College of Medicine (BCM) Physician Assistants and Nurse Practitioners at the Texas Medical Center campus to be part of a dynamic and collaborative multidisciplinary medical team who will care for hospitalized patients with a variety of gastrointestinal and liver disorders. This position offers a unique experience on our in-patient service lines for an individual who wishes to pursue a career in pediatric gastroenterology, hepatology and nutrition, build relationships with patients/families and be immersed in a highly educational environment with frequent opportunities to attend research lectures, pathology conference, and obtain CME through GI grand rounds and fellows workshops. The Division of GHN at TCH is currently ranked the #5 program in the country by US News and World Report and its faculty are internationally recognized clinical experts and researchers. As the largest children's hospital in the country, TCH is a major quaternary referral center for some of the rarest GI and liver disorders, allowing for unparalleled development of pediatric GI and hepatology expertise. Currently, three gastroenterology, hepatology, and nutrition in-patient service lines care for 20 - 35 patients per day with a wide range of gastrointestinal, liver, and nutritional disorders including but not limited to: inflammatory bowel disease, short bowel syndrome/ intestinal failure, gastrointestinal motility disorders, liver diseases, liver transplantation, and pancreas and bile duct abnormalities. Management of nutritional support in the form of enteral tube feeding and parenteral nutrition is also provided. The service line also performs a variety of diagnostic and therapeutic endoscopic procedures. The physician assistant/ instructor will work alongside faculty colleagues, pediatric gastroenterology and transplant hepatology fellows, pharmacists, dieticians, social workers and pediatric residents to care for this pediatric patient population in both the acute care and ICU settings. We seek individuals who are organized and detail-oriented and can work well with staff and faculty from different departments to coordinate outstanding patient care.
