FLSA Status Non-Exempt Job Role Summary The Team Captain RN and Triage Nurse RN are positions that allow for a collaborative approach to the patient's healthcare in a multitude of ways. Team Captain RN - This role is for the nurse responsible for the overall care of patients assigned to their team's providers. This includes risk assessment and coordination of care for high risk patients, tracking quality metrics, delivering elements of patient education, and serving as the principal point of contact for patients and other care providers outside of face-to-face appointments. The Team Captain supervises and directs the activities of team nurses and support staff. Triage Nurse RN - Triage is an interactive process between the nurse and patient that occurs over the telephone or face-to-face and involves identifying the nature and urgency of client healthcare needs and determining the appropriate disposition. The role of the triage nurse is to assess the patient, provide patient education, and crisis intervention. The goal of telephone triage services is to direct the caller to the appropriate level of care or service in a safe and timely manner in combination with providing self-care advice and direction. Works with team providers and team members to provide patient education regarding diagnoses, medications, and wellness and self-management skills. Provides nurse visits for patients assigned to the team, and ensures that team members provide care within their scope of practice. Initiative & Growth: Demonstrates a self-directed work effort aligned with team goals. Works collaboratively in the care and care management of patients and their families. Overview Coordinates and provides high quality multidisciplinary health services for patients in the Hospice Care Program consistent with Agency philosophy, policy, goals and objectives, and Standards of Nursing Practice. Works under moderate supervision. SCHEDULE: Friday, Saturday, Sunday 2:00pm-2:00am • Assesses the physical, social and psychological needs and living conditions of the patient through professional knowledge and skills of observation, interviewing and physical examination. • Travels to patients’ homes and/or other facilities with varying environments (e.g., elevated buildings, walk-ups, care facilities, single/multiple family homes, presence of pets, etc.) using approved transportation options to deliver direct care to the patient. • Implements care plan and revisions by ongoing assessment, planning, coordination, counseling, teaching and supervision.
