Rutgers, The State University of New Jersey, is seeking a Health Care Case Manager for Newark Center City office for the Child Health Program within the School of Nursing. The Heath Care Case Manager (HCCM) functions to support the Department of Children and Families (DCF)/Child Protection and Permanency (CP&P) in meeting the health care and protection needs of children in out-of-home placement, in collaboration with other staff of the Child Health Unit in that local office. Among the key duties of this position are the following: Upon entry into out-of-home placement, assists with enrollment of a child in a managed care entity and identification of a medical home (primary care provider), and informs the child, caregiver and DCF/CP&P, as needed. Assesses a child’s heath care status and needs by retrieving a complete health history through obtaining and reviewing all medical records and interviewing past providers and caregivers; forwards all information to the medical home and other health providers as needed. Schedules an Initial Physical Health Evaluation, in consultation with the caregiver and DCF/CP&P, reviews the preliminary and final reports, and ensures that all recommendations for health care are completed. Develops a child health care plan in collaboration with the child’s medical home, as well as involved specialists, caregivers, other community service providers, and when appropriate, DCF/CP&P. Links the child and caregiver with appropriate health resources and community services. Assures that the child is scheduled for and attends timely EPSDT visits and specialty appointments, reviews all reports, and ensures all necessary reports are forwarded to the medical home.
