They also work closely with the Director of Staff Development, Infection Prevention and Control Officer as well as the Quality Improvement team to ensure that all nursing staff is educated in the principles of wound prevention and treatment. The Wound Care Nurse implements treatment modalities as assigned. Responsibilities include but are not limited to: Assess skin condition of new admissions and readmissions using the standardized assessment tool Uses the Braden Scales to predict pressure sore risk as needed Implements treatment plan for patients and residents as appropriate and administers treatments Implements and evaluates the plan of care reflecting appropriate nursing and medical diagnosis, goal development, outcome, criteria and target dates. Keeps patient and resident plans of care updated reflecting changes in condition and responses to treatment or education Appropriately documents on patients and residents receiving treatments, reviewing previous documentation. Completes initial and Weekly skin Impairment forms Participates as an active member of the Interdisciplinary team, including the Wound Care Team for Weekly Wound Rounds and is part of the Wound Committee; completes the weekly wound report.
