Description: Work Schedule: 60% FTE, day/evening, 12-hour shifts, every other weekend rotation plus required on-call responsibilities during the off-hours (evenings/nights) with a one-hour response time to travel to patient’s home. Scheduled holiday rotation. This position is eligible for shift and weekend differentials and mileage reimbursement. Hours are based off the operational needs of the department. Perform comprehensive patient assessments, evaluating physical, functional, psychosocial, and cognitive status while monitoring response to treatment in the home environment. Support safe transitions of care, identifying post-acute needs and coordinating referrals, services, and equipment in collaboration with the HBHC team.
