This hybrid role includes both in-clinic and remote work and focuses on triage nursing, requiring the ability to manage a high call volume, coordinate care efficiently, and multitask in a fast-paced environment while ensuring exceptional patient care. In this role, you will work closely with physicians, advanced practice providers, and other healthcare professionals to optimize patient outcomes through patient education, symptom management, and care coordination. This position will also participate in the assessment and management of geriatric syndromes, including cognitive decline, falls, and polypharmacy concerns. Job Description Primary Duties & Responsibilities: Utilizes the nursing process in assessing, planning, implementing and evaluating a plan of patient care. Provides patient care, which may include administering medication, performing procedures and monitoring patient status in accordance with approved nursing practice.
