Job Summary: This role is a Registered Nurse that directly supports primary care providers in Electronic Health Record (e.g. MyChart) message management. The RN will work under the supervision of the Clinic Nursing Leadership team with close collaboration with the clinic medical director and primary care department physician leadership to optimize EHR message management through the electronic health record (EHR) system to support the ever-growing population of patients who rely on EHR messaging to access health care. The RN is the first line of contact to patients in response to EHR messages. The role is to respond to all EHR messages by offering clinical guidance, triage to higher level of care, and patient education prior to heightening to provider level, if indicated. This role will also be able to offer development in optimizing patient communication through electronic platforms through patient education on best use of EHR messaging, phone call, virtual visits, and face-to-face visits. Job Duties: Utilizes the nursing processes to provide holistic, compassionate, safe, high quality, and population-based care. Serves as an advocate and resource to patients who seek care via EHR messaging. Reviews all in-basket messages for all primary care providers daily (whether providers are in clinic or off) to “mature” each message before routing to provider or covering provider, when indicated. Turn EHR messages into triage encounters when indicated as part of “maturing” the encounter. Meet up to weekly for with clinic medical director or designated physician “advisor” for education on case review and to share insight on effective management of in-baskets. Collaborates with Behavioral Health Clinician to coordinate behavioral and mental health patients who frequently utilize EHR messaging to drive care management. Collaborates with clinic care coordination team to develop plans to best assist patients with high EHR messaging utilization. Provide patient education and care coordination on best ways to seek care through supported messaging platforms. Provides evidence-based education to patients/families related to clinical care. Makes follow up patient phone calls to ensure patient feels confident in their care direction, stimulated from EHR message inquiries. Assists to coordinate follow-up appointments with primary or specialty care. Knows the key staff available to help patients with financial and emotional support; arranges for patients to meet with these resource people if needed. Serve as a contact for patients, providers, and other customers. Other duties as assigned. If you have the following qualifications, we would love to talk to you!
