You will collaborate with the patient, caregivers, clinical manager, case managers, and physician(s) to complete the patient care planning at the onset of care. You will provide skilled interventions and instructions to the patient and caregivers to provide optimum symptom control and patient comfort. You will function as a liaison between disciplines during the admission process. ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES Patient Care 1. Completes an initial, comprehensive assessment of patient and family to determine hospice needs. Provides a complete physical assessment and history of current and previous illness(es). 2. Provides professional nursing care by utilizing all elements of the nursing process. 3. Assesses and evaluates patient’s status by: A. Writing and initiating plan of care B. Evaluating patient and family/caregiver needs C. Participating in revising the plan of care as necessary 4. Initiates the plan of care and makes necessary revisions as patient status and needs change. 5. Uses health assessment data to determine nursing diagnosis. 6. Develops a care plan that establishes goals, based on nursing diagnosis and incorporates palliative nursing actions. Includes the patient and the family in the planning process. 7. Initiates appropriate preventive and rehabilitative nursing procedures. Administers medications and treatments as prescribed by the physician in the physician’s plan of care. 8. Teaches the patient and family/caregiver self-care techniques as appropriate. Provides medication, diet and other instructions as ordered by the physician and recognizes and utilizes opportunities for health counseling with patients and families/caregivers. Works in concert with the interdisciplinary group. 7. Provides and maintains a safe environment for the patient. 8. Assists the patient and family/caregiver and other team members in providing continuity of care. 9. Works in cooperation with the family/caregiver and hospice interdisciplinary group to meet the emotional needs of the patient and family/caregiver. 10. Attends interdisciplinary group meetings. Supervises ancillary personnel and delegates responsibilities when required. 4. Care Is usually provided at the person’s home, or facility. Nurse Next Door affiliated home care aides shall be selected on the basis of such factors as a sympathetic attitude towards the care of the sick, ability to carry out directions, maturity and ability to deal effectively with the demands of the job. All affiliated Home care aides employed by Nurse Next Door to provide in-home care services will be 18 years old (or if under 18 years of age be a high school graduate or enrolled in a vocational education program) and have sufficient communication and language skills to enable them to perform their duties and interact effectively with clients and other agency staff. Home care aides must meet all registration, criminal background checks and training requirements as specified by the State of California Department of Social Services Home Care Services Bureau and Nurse Next Door policies. Essential Job Functions/Responsibilities Responsibilities of the home care aide include, but are not limited to, the following- · Execute the client’s care plan as outlined by the Care Designer. · Help clients with their ADLs (Activities of Daily Living) that are within the scope of practice for a Registered Health Care Aide. · Assisting clients with personal hygiene such as skin, mouth, hair care, bathing and dressing. · Assisting clients in and out of bed, transfers, positioning and assisting with ambulation. · Assisting with prescribed exercises which clients and aides have been taught by appropriate health personnel. · Meal planning and preparation, including therapeutic diets, and assisting clients with eating. · Assisting clients to the bathroom or in using commodes, bedpans or urinals and incontinence care. · Performing household services including housekeeping, laundry, transportation, correspondence, making telephone calls, shopping for personal care items or groceries, and companionship. · Reporting changes in the client's condition and needs to the administrator or care designer. · Completing records regarding services performed. · Driving to appointments or to run errands. · Accurately records all hours worked. · Remains alert throughout the shift in case client requires assistance, responds immediately to any such needs by providing the assistance required, or calling the Company for guidance. · Participating in all required entry-level and annual training requirements. · Maintaining an active home care aide registration with the State of Ca. Dept. of Social Services Home Care Services Bureau. · Other duties as assigned by the administrator or designee. Position Qualifications · At least 18 years of age. · Current registration with the State of California Home Care Aide Registry. We Provide paid training and help you with the registration process. · Completion of a criminal background check and disclosure of any criminal history or evidence of exemption. · Evidence of entry-level training or plan to complete paid training prior to client care and in accordance with CA regulations. · Evidence of tuberculosis testing and verification that the home care aide does not have active tuberculosis. · Ability to read and follow written instructions and document care given. · Self-directing with the ability to work with little direct supervision. · Empathy for the needs of the ill, injured, frail and the impaired. · Demonstrates tact, patience, and good personal hygiene. · Valid driver’s license and auto insurance and/or reliable transportation required. Physical Demands · Regularly required to sit, use hands or fingers, handle, or feel and talk or hear. · Required to reach with hands and arms. · Required to stand, walk, climb or balance, stoop, kneel, crouch, or crawl, and taste or smell. · Must be able to lift and or move up to 10 pounds, frequently lift and or move from 25 up to 50 pounds. · Specific vision abilities required by the job include close vision, distance vision, peripheral vision, depth perception and ability to adjust focus.
