Job Description Nemours Children’s Health is seeking an experienced pediatric telephone triage nurse to support our primary care practices in Delaware, Pennsylvania and Florida. This is a fully remote position that requires every other weekend and every other holiday. This is a part time (0.5 FTE) role that will be 20hr/ wk ). Weekdays Hours for the role range from: 7:00am-5pm (D) 4:00pm-12am (AH) Overnight 12:00pm-8:30am No permanent night shifts are included for this role, but must be available to cover up to 3 overnight shifts per year Weekends 7:00am-3:30pm 3:30pm-12:00am Overnight 12:00pm-8:30am Primary Function The Triage Nurse role provides provide primary care patients telephone access to a highly skilled and trained registered nurse who accurately answers their questions and addresses their medical concerns during hours when the primary care offices are open. To provide first line response to patient questions and concerns with escalation to the primary care offices when necessary. To ensure continuity of care, communicates the patient’s condition and plan of care to other health team members. Essential Functions Accurately assesses each patient’s condition by actively listening to callers and asking probative questions. Identifies emergent situations and effectively transfers life threatening calls to appropriate agencies per protocol. Provides appropriate and courteous telephone advice including plan of care and patient education to callers according to approved protocols. Appropriately and accurately documents initial and ongoing telephone conversations with documentation being completed immediately after the call ends. Places outbound follow up calls to caregivers as needed. Accurately communicates information to physician on call as needed either over the telephone or by routing the necessary information to the physician through EPIC. Works autonomously while adhering to organizational standards and core values. Attends and participates in staff meetings and required departmental meetings. Meets the needs of the department by flexing hours as needed based on seasonal changes and call volume. This may include morning, evening, weekend, and midnight shifts. Meets unit expectations for call volumes, call duration and customer service expectations. Appropriately utilizes and maintains Nemours supplied equipment; ability to troubleshoot technology issues to ensure uninterrupted service to patients. Additional miscellaneous duties and responsibilities, as may be assigned from time to time by employee’s supervisor. Job Requirements Bachelor’s Degree strongly preferred. Associates Degree prepared candidates must be actively enrolled in a BSN program. Upon hire, the Kids Health On Call nurse must have an active unrestricted Delaware Registered Nurse license to start. Active Pennsylvania and Florida Registered Nurse Licenses must be obtained within 90 days of hire date. BLS from American Heart Association required for all nursing roles. Nurses residing in an interstate compact state (DE, NJ or MD) who have a multistate license for their state of residence will not need to obtain separate PA or FL licensure. Nurses residing in PA will continue to require separate DE and FL licensure until PA begins full participation in the interstate compact. The KHOC associate must not work autonomously until all licensure is obtained and verified- they can shadow and train with a KHOC preceptor, but cannot provide medical advice for patients receiving primary care services in states for which the associate has not yet obtained licensure. 3-5 years of job related experience. I. Qualifications Skilled Nurse from an Accredited program currently licensed and in good standing with the Pennsylivna State Board of Nursing. One to 1 years experience in nursing in an acute care setting or 1-year experience in home health, hospice or comparable experience is preferred. Desire to learn and demonstration of aptitude for the hospice philosophy and comfort care required. II. Responsibilities Nurse Manager, works in conjuction with Hospice Executive Director and the Interdisciplinary Team to: · provide hands-on clinical nursing, interacting with patients, family, physicians, Interdisciplinary Team members, and other health care professionals involved in the patient’s care. · assess and coordinate a pain/symptom management program formulated with the participation of family and patient, attending physician and the Interdisciplinary Team, utilizing pharmacological and non-pharmacological methods of pain/symptom control to enhance quality of life. · assist in facilitating freedom of choice, relief of suffering and death with dignity for patients facing end of life issues. · provide support and education for families based on the hospice philosophy of care. Care includes physical, psychological, spiritual, and financial comfort. · participate in Interdisciplinary Team meetings and up-dates the Plan of Care (POC) for assigned patients based on identified problems and needs. · ensure all services provided are in accordance with the patient’s POC. · ensure the POC involves the active participation of the patient, family, PCP, and the Interdisciplinary Team · educate patient and family on the disease process and expectation related to the disease progression. · assist with CQI activities. · work as a team member at all times and uphold the Hospice Code of Ethics · provide care to manage patient symptoms. · utilize the medication formulary whenever possible for the management of patient symptoms. III. Orientation and TRAining Tender Care Hospice will provide orientation and training, as necessary, to assist the Nurse Manager in performing the responsibilities listed above. Training may be in-house or outside seminars. Evidence of orientation and training will become part of the personnel file. IV. On Call The Nurse manager will be on call according to scheduling. During the hours when on call, the Nurse Manager is expected to follow On Call Protocol and to respond to all pages within 20 minutes. V. Accountability Reports directly to the Hospice Executive Director. 
Job Description Nemours is seeking a Registered Nurse for our general pediatrics stepdown unit to join our team in Wilmington, Delaware . This position is FULL-TIME, 72 hours biweekly, 12-hr rotating shifts, with every third weekend and holiday rotation . 3 East is a surgical intermediate unit with a high population of patients requiring post-operative care for urological issues, traumatic injuries, and general surgery. 3 East also cares for patients with acute and end stage renal disease and patients requiring an intermediate level of nursing care due to their tracheostomy and/or ventilator needs. Each patient room is equipped with oxygen, compressed air, and suction along with state-of-the-art cardiorespiratory bedside monitors and computers that enable the healthcare team to access a patient’s electronic medical record. The patient care complexity directly relates to the wide range of patient care skills required for this diverse patient population. The goal of this unit is to provide high-quality, cost-effective patient care. The Synergy model is used in making appropriate patient assignments based on patient and family needs and nursing skill set. Special equipment used on this unit includes infusion pumps, syringe pumps, feeding pumps, BiPap respirators, Oxygen delivery methods, PCA & epidural pumps, wound vacutainers, ventilators and specialty beds. Nursing competencies include: chest tube management, pain management, blood product administration, urinary catheterization, tracheostomy care, ventilator management, and central line care and management. Qualifications We are looking for professional and dedicated nurses passionate about pediatrics BSN preferred; if have associate's degree must receive BSN within three years Pediatric experience preferred At least 6 months of nursing experience required Bilingual Preferred Must be eligible for or have current DE nursing license.
