The Nurse Practitioner will be responsible for conducting client assessments, developing treatment plans, and providing ongoing care to our clients. What tasks are required? Perform initial and on-going assessment of client’s medical, physical, and psychosocial status. Prescribe and monitor medication as necessary for clients in recovery. Obtain and evaluate health and medical history; perform a physical exam, conduct preventive screening procedures based on age and history; Identify medical and health risk. Collaborate with the multidisciplinary team to provide holistic care to clients. Complete accurate and timely documentation of observations, interventions, plan of care, referrals, follow-up, and progress in electronic health records. Participate in staff meetings and training sessions as required. What we need from you? Active Nurse Practitioner license in the state. 2 years of experience working with clients with substance abuse disorders preferred. Experience prescribing medication-assisted treatment (MAT) preferred. Strong communication and collaboration skills. Ability to work independently and as part of a team.
