Job Description Our 3 East unit is seeking a Nursing Assistant to join our team in Wilmington, DE! The nursing assistant role is responsible for assisting with general activities of the patient care unit and assist the health care team with performance of patient care activities, under the direction and guidance of a Registered Nurse. Position is full-time, 60 hrs every 2 wks, rotating day and night shifts with weekend and holiday requirement. 3 East is a surgical intermediate unit with a high population of patients requiring post-operative care for urological issues, traumatic injuries, and general surgery. 3 East also cares for patients with acute and end stage renal disease and patients requiring an intermediate level of nursing care due to their tracheostomy and/or ventilator needs. Each patient room is equipped with oxygen, compressed air, and suction along with state-of-the-art cardiorespiratory bedside monitors and computers that enable the healthcare team to access a patient’s electronic medical record. The patient care complexity directly relates to the wide range of patient care skills required for this diverse patient population. The goal of this unit is to provide high-quality, cost-effective patient care. The Synergy model is used in making appropriate patient assignments based on patient and family needs and nursing skill set. Special equipment used on this unit includes infusion pumps, syringe pumps, feeding pumps, BiPap respirators, Oxygen delivery methods, PCA & epidural pumps, wound vacutainers, ventilators and specialty beds. Nursing competencies include: chest tube management, pain management, blood product administration, urinary catheterization, tracheostomy care, ventilator management, and central line care and management. Essential Functions Ensures an adequate stock of all necessary equipment and supplies in designated areas at established par levels. Ensures a clean and safe patient care unit. Assists patients with activities of daily life (bath, feed, play, etc.) and provides other patient care as directed. Assures all supplies are accurately charged. Contacts pharmacy, dietary, and other departments as designated by the Registered Nurse. Collects patient specimens (blood, urine, stool, other per policy), paying close attention to patient safety, comfort and accuracy of information. Uses Hospital Information Systems as required for patient care, (e.g. documenting vital signs and patient care activities.) Accurately relays information from physicians, nurses, laboratory personnel and others. Transports patients and delivers reports, supplies, etc., to other patient units or Institute locations as needed. Courteously and professionally covers patient care unit front desk as needed. Job Description Nemours is seeking an Advanced Practice Provider to join our Nemours Children’s Perinatal Program in Wilmington, DE. The Nemours Fetal Care Center and Advanced Delivery Unit are expanding the scope of service to include comprehensive prenatal, obstetrical and postpartum care for women whose unborn child has been diagnosed with a birth defect or complex medical condition that will require immediate multidisciplinary pediatric care in the neonatal period. In addition to currently available prenatal imaging to diagnose these conditions, we are also expanding invasive diagnostics and fetal intervention procedures. The NFCC-APP will work with the Maternal-Fetal Medicine physicians, Obstetricians and the entire multidisciplinary team to provide full scope maternal care including prenatal care, postpartum care, inpatient care, assist in fetal procedures in the outpatient setting and in the operating room, and assessment of fetal well-being. In this role, the qualified NFCC-APP will play an integral role in the care and support of these patients throughout their high-risk pregnancy journey into the postpartum period. Job Description The Perioperative Services Lead Surgical Technologist is responsible for providing guidance, clinical expertise, and mentoring to members of the health care team within the specialty while optimizing available resources to achieve optimal patient outcomes. Will schedule and obtain laboratory specimens as ordered by the physician or Medical Director. Will assist with follow-through or may initiate precautionary measures in case of suspected infection in cooperation with the Infection Control Nurse. Assign personnel and make staff adjustments according to needed coverage to meet resident needs. Acquire replacement staff when an employee calls in; assist with scheduling as needed. See that personnel are oriented to duties assigned to them and evaluate job performance of staff under their supervision. Maintain an open line of communication with the Director of Nursing, Medical Director, Nurse Managers, other departments and consulting agencies of residents' status and needs and staff and unit needs. Responsible for notifying residents' primary contact if residents' status changes. Complete and distribute daily census sheets. Assist with updating care plans as indicated. Ensure that established policies and procedures including dress codes are followed by departmental personnel. Perform any other pertinent duties as assigned by my supervisor. Uses health assessment data to determine nursing diagnosis. 6. Develops a care plan that establishes goals, based on nursing diagnosis and incorporates palliative nursing actions. Includes the patient and the family in the planning process. 7. Initiates appropriate preventive and rehabilitative nursing procedures. Administers medications and treatments as prescribed by the physician in the physician’s plan of care. 8. Counsels the patient and family in meeting nursing and related needs. 9. Provides health care instructions to the patient as appropriate per assessment and plan. 10. Assists the patient with the activities of daily living and facilitates the patient’s efforts toward self-sufficiency and optional comfort care. 11. Acts as Case Manager when assigned by Clinical Supervisor and assumes responsibility to coordinate patient care for assigned caseload. Communication 1. Completes, maintains and submits accurate and relevant clinical notes regarding patient’s condition and care given. Records pain/symptom management changes/outcomes as appropriate. 2. Communicates with the physician regarding the patient’s needs and reports changes in the patient’s condition; obtains/receives physicians’ orders as required. 3. Communicates with community health related persons to coordinate the care plan. 4. As a mandatory reporter, reports failure to comply with the requirements of the Dept. of Health as required within 14 calendar days, using Dept. of Health forms. 5. Reports suspected abandonment, abuse, financial exploitation, or neglect of a person in violation of the department of social and health services and the proper law enforcement agency. Reports must be submitted immediately when the reporting person has reasonable cause to believe that abandonment, abuse, financial exploitation, or neglect of a vulnerable adult has occurred. 6. Teaches the patient and family/caregiver self-care techniques as appropriate. Provides medication, diet and other instructions as ordered by the physician and recognizes and utilizes opportunities for health counseling with patients and families/caregivers. Works in concert with the interdisciplinary group. 7. Provides and maintains a safe environment for the patient. 8. Training may be in-house or outside seminars. Evidence of orientation and training will become part of the personnel file. IV. On Call The Nurse manager will be on call according to scheduling. During the hours when on call, the Nurse Manager is expected to follow On Call Protocol and to respond to all pages within 20 minutes. V. The professional nurse builds from the resources of the community to plan and direct services to meet the needs of individual and families within their homes and communities. ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES Patient Care 1. Completes an initial, comprehensive and ongoing comprehensive assessment of patient and family to determine hospice needs. Provides a complete physical assessment and history of current and previous illness(es). 2. Provides professional nursing care by utilizing all elements of nursing process. 3. Assesses and evaluates patient’s status by: A. Writing and initiating plan of care B. Regularly re-evaluating patient and family/caregiver needs C. Participating in revising the plan of care as necessary 4. Initiates the plan of care and makes necessary revisions as patient status and needs change. 5. Uses health assessment data to determine nursing diagnosis. 6. Develops a care plan that establishes goals, based on nursing diagnosis and incorporates palliative nursing actions. Includes the patient and the family in the planning process. 7. Initiates appropriate preventive and rehabilitative nursing procedures. Administers medications and treatments as prescribed by the physician in the physician’s plan of care. 8. Counsels the patient and family in meeting nursing and related needs. 9. Provides health care instructions to the patient as appropriate per assessment and plan. 10. Assists the patient with the activities of daily living and facilitates the patient’s efforts toward self-sufficiency and optional comfort care. 11. Acts as Case Manager when assigned by Clinical Supervisor and assumes responsibility to coordinate patient care for assigned caseload. Communication 1. Completes, maintains and submits accurate and relevant clinical notes regarding patient’s condition and care given. Records pain/symptom management changes/outcomes as appropriate. 2. Communicates with the physician regarding the patient’s needs and reports changes in the patient’s condition; obtains/receives physicians’ orders as required. 3. Communicates with community health related persons to coordinate the care plan. 4. As a mandatory reporter, reports failure to comply with the requirements of the Dept. of Health as required within 14 calendar days, using Dept. of Health forms. 5. Reports suspected abandonment, abuse, financial exploitation, or neglect of a person in violation of the department of social and health services and the proper law enforcement agency. Reports must be submitted immediately when the reporting person has reasonable cause to believe that abandonment, abuse, financial exploitation, or neglect of a vulnerable adult has occurred. 6. Teaches the patient and family/caregiver self-care techniques as appropriate. Provides medication, diet and other instructions as ordered by the physician and recognizes and utilizes opportunities for health counseling with patients and families/caregivers. Works in concert with the interdisciplinary group. 7. Provides and maintains a safe environment for the patient. 8. Assists the patient and family/caregiver and other team members in providing continuity of care. 9. Works in cooperation with the family/caregiver and hospice interdisciplinary group to meet the emotional needs of the patient and family/caregiver. 10. Attends interdisciplinary group meetings. Additional Duties 1. Participates in on-call duties as defined by the on-call policy. 2. Ensures that arrangements for equipment and other necessary items and services are available. 3. Supervises ancillary personnel and delegates responsibilities when required. 4. Assumes responsibility for personal growth and development and maintains and upgrades professional knowledge and practice skills through attendance and participation in continuing education and inservice classes. 5. Fulfills the obligation of requested and/or accepted case assignments. 6. Actively participates in quality assessment performance improvement teams and activities. POSITION QUALIFICATIONS 1. Graduate of an accredited school of nursing 2. Registered Nurse, with a License in good standing to practice. 3. Registered nurses shall have a minimum of (a) one (1) year of experience as a professional nurse within the last three (3) years; OR have a baccalaureate degree in nursing and minimum of two (2) years’ experience, at least one of which is in the area of public health, home care, or hospice nursing is preferred. 4. Management experience not required. Responsible for supervising hospice aides. 5. Once an offer of employment is made, it is contingent upon satisfactory references, as requested, and criminal background checks by regulation. 6. Excellent observation, verbal and written communication skills, problem solving skills, basic math skills; nursing skills per competency checklist. 7. Prolonged or considerable walking or standing. Able to lift, position and/or transfer patients. Able to lift supplies and equipment. Considerable reaching, stooping bending, kneeling and/or crouching. Visual acuity and hearing to perform required nursing skills. 8. Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order. 9. Possesses and maintains CPR certification (may not be internet based unless the demonstration of skills was hands on and observed by a certified trainer)
