Serves as a single point of contact for referring physicians, clients, caregivers, and human service organizations to provide resources and assistance with behavioral health care services offered within South Shore Health System and its partnering healthcare community. The BHN will provide brief interventions in Acute Care, primary care and other community settings, educate clients on the importance of MH services, and navigate clients to a long-term provider. Serves as a liaison between clients, healthcare providers and human service agencies to reduce barriers to care and that assure clients receive the care they need when they need it. The BHN will focus on behavioral health coaching (e.g. brief intervention), removing barriers to care (stigma, insurance status), navigating to long-term behavioral healthcare, educating and empowering the clients on behavioral health intervention and screening, and advocating for and coordinating client care with PCPs and behavioral health providers. ***At this time we can only consider RN's with Emergency Department and/or inpatient behavioral health/psychiatric patients and or RN Case Management Experience*** This position is on-site (5-days/week) with no option work remotely. Prior experience working in an acute care hospital: inpatient psychiatric or emergency department and/or Nurse Case Manager Behavioral Health. Compensation Pay Range: $93,000.00 - $175,000.00 ESSENTIAL FUNCTIONS 1. Identify clients who could benefit from Behavioral Health services using referrals acute care providers, primary care providers (PCP), behavioral health providers, or other community agencies. 2. Connect with referred clients in the acute care setting, during PCP visits or in other community settings skillfully and effectively. 3. Assess presenting problems efficiently, and developing an initial treatment plan with the client or family seeking treatment. 4. Assist clients in understanding their diagnosis, treatment options, and the resources available, including educating eligible clients about appropriate community services, clinical research studies, and technologies. 5. Provide brief behavioral health interventions in the form of Individual, Family, or Group treatment. Possesses knowledge of substance abuse assessment, recovery principles, behavioral activation, and chronic disease self-management. 6. Complete intake of high-risk clients, working with the client, family and other members of the healthcare team as needed, to assess client’s behavioral health needs, physical health, support system, financial resources, and available community and government resources. Refer to crisis services as needed. 7. Determine with the client specific objectives, goals, and measures that meet the client’s needs and that have been identified through assessment. Utilize the needs assessment to find a mental health home for the client and provide warm hand-offs to long-term mental health providers. 8. Serve as an essential link between clients and all other care providers. Represents client/family by intervening, negotiating and promoting their concerns. Receives/transcribes physician orders and sees that all orders are carried out promptly and accurately. 2. Receives and places calls to physicians/consultants. 3. Documents in progress notes and narcotic log all medication and treatment record. 4. Completes progress notes on a timely basis addressing pertinent issues and changes in patient status to ensure continuity of care. 5. Ensures progress notes reflect plan of care and established goals. 6. Administers and records all treatments according to physician orders. 7. Assesses and recognizes changes in the condition of residents during his/her shift and reports changes to immediate supervisor. 8. Properly orders and receives medications from pharmacy. 9. Properly administers, stores, and charts all routine and PRN medications, timely and accurately. 10. Understands and practices proper procedure for administering, documenting, and removal of Schedule II, III, and IV drugs. 11. Correctly performs and documents narcotic count. 12. Understands procedure for incorrect narcotic count. 13. Maintains treatment and medication cart with all supplies needed to carry out orders. 14. Administers parental, intramuscular, and sub-cutaneous injections. 15. Performs peripheral IV insertion, administration of fluids and medications and monitors site and patient condition for complications. Performs dressing changes and flushes per protocol. 16. Monitors meal services for completeness, appropriateness, and fluid intake. 17. Completes general assessments and risk assessment, as required. 18. Collects, properly labels, and stores all specimens – completes lab slips as needed. 19. Interprets lab data and reports significant results to physicians. 20. Administers 2 step PPD’s on admission. Records on appropriate form. 21. Able to assess skin integrity using established protocols. 22. Follows protocols for treatment and prevention of pressure ulcers. 23. Able to assess respiratory status of patient utilizing pulse oximetry, lung sounds, vital signs, and need for suctioning. 24. Able to administer oxygen via mask or cannula with concentrator, tank, or liquid oxygen. 25. Able to assess cardiac status utilizing vital signs, color, respiratory status, pulse deficit, skin condition, and presence or absence of pain. 26. Performs procedure for NGT, GT, and JT feeding via bolus or infusion pump. Performs flushes and dressing changes per physician orders. 27. Able to adequately and safely perform all procedures documented on the skills check list. 28. Trains ad orients new staff. 29. Addresses problems with employees as they arise in a fair, consistent, and effective manner. 30. Coaches, encourage, and support staff; and praises employees for a job well done. 31. Answers questions regarding policies and procedures. 32. Leads by example.
