We are currently seeking Wound Care Nurse Practitioners to provide wound services to patients living in long-term care facilities. WOUND NP ROLE AND RESPONSIBILITIES: Provide wound care services for long-term care patients residing in skilled nursing facilities. Prescribe appropriate medications, including controlled substances in accordance with NH Law. Order, review, and/or follow-up with treatments and diagnostic testing. WOUND NP QUALIFICATIONS AND EDUCATION REQUIREMENTS Current, unrestricted NP license in the state of New Hampshire. Board Certification. DEA licensure (Optional) Clinical background in adult, family, or geriatric specialties. Clinical background of wound care; including but not limited to chronic, debilitating, stalled, or infected wounds Must be comfortable with diagnosing and treating diabetic, arterial, venous, and mixed etiology wounds or Kennedy Ulcers. Must be comfortable staging pressure injuries according to the NPIAP Must be familiar with wound care best practice and advanced wound care products; collagens, alginates, foams, silvers, etc. Must have experience with wound vacs and other DME, such as pressure relieving/ reducing devices Strong preference to having wound certification or be open to getting certified Experience working in a long-term care setting. Ability to work with an EMR. Ability to work within an interdisciplinary care team to coordinate patient care. State Certification: Certified by the New York State Department of Education as a Nurse Practitioner with current registration as follows: Is a graduate of New York State registered Nurse Practitioner Program or equivalent OR is certified as a Nurse Practitioner by a national association via examination. Has completed a pharmacy component of not less than the required semester hours or the equivalent AND has been instructed in New York State and Federal Laws relating to prescriptions and recordkeeping (3-hour course). Has a Collaborative Practice Agreement with a physician and designated protocols if less than 3600 hours experience as a practicing Nurse Practitioner. OR Has a Collaborative Relationship with a physician based on employment in clinical practice in the NP role at Mount Sinai Hospital. If there is a documented work history as a Nurse Practitioner with greater than 3600 practice hours. Please note all NPs regardless of their status will be placed on an FPPE initially) Education: Master's degree program designed to prepare a NP to diagnose, treat, and prescribe for a patient’s condition that fall within their specialty area of practice or the successful completion of an approved supplemental education program, i.e., post-Masters NP certificate.
