We are currently seeking Wound Care Nurse Practitioners to provide wound services to patients living in long-term care facilities. WOUND NP ROLE AND RESPONSIBILITIES: Provide wound care services for long-term care patients residing in skilled nursing facilities. Prescribe appropriate medications, including controlled substances in accordance with KY Law. Order, review, and/or follow-up with treatments and diagnostic testing. Communicate with the nursing home staff on a regular and as needed to discuss changes in condition. and/or the care plan. Collaborate with the primary care physician. WOUND NP QUALIFICATIONS AND EDUCATION REQUIREMENTS Current, unrestricted NP license in the state of Kentucky. Board Certification. DEA licensure (Optional) Clinical background in adult, family, or geriatric specialties. Clinical background of wound care; including but not limited to chronic, debilitating, stalled, or infected wounds Must be comfortable with diagnosing and treating diabetic, arterial, venous, and mixed etiology wounds or Kennedy Ulcers. Must be comfortable staging pressure injuries according to the NPIAP Must be familiar with wound care best practice and advanced wound care products; collagens, alginates, foams, silvers, etc. Must have experience with wound vacs and other DME, such as pressure relieving/ reducing devices Strong preference to having wound certification or be open to getting certified Experience working in a long-term care setting. Ability to work with an EMR. Ability to work within an interdisciplinary care team to coordinate patient care. As a MAT Nurse Practitioner , you will work alongside correctional officers and other medical professionals to provide support to the patients within the facility struggling with substance abuse. Initiate, continue, adjust, and monitor Suboxone therapy in accordance with evidence-based guidelines, DEA regulations, and correctional health policies. Manage induction, stabilization, maintenance, and tapering phases of MAT as clinically indicated. Collaborate closely with correctional healthcare staff, custody staff, mental health providers, and substance use counselors to ensure coordinated, safe, and effective care. Monitor patient adherence, response to treatment, and potential diversion risks within a correctional setting. Provide patient education on OUD, MAT, relapse prevention, overdose risk, and continuity of care upon release. Participate collaboratively with an interdisciplinary team to create individualized treatment plans that address medical, behavioral health, and social determinants of health. Ensure accurate, timely, and compliant documentation in the electronic health record, including MAT-specific reporting and regulatory requirements. Participate in discharge planning and continuity-of-care coordination, including referrals to community-based MAT providers upon release. Maintain compliance with state, federal, and facility regulations related to controlled substances, MAT delivery, and correctional healthcare standards. Participate in quality improvement initiatives, clinical audits, and program development related to OUD and MAT services. Job Details Medical Group l Dayton l Full-Time l First Shift Responsibilities & Requirements Advanced Practice Provider (APP) is a provider of heart & vascular health care services who synthesizes and utilizes evidence-based practice, research, and current clinical knowledge. Working in collaboration with a multidisciplinary health care team. The APP demonstrates an advanced level of medical and/or nursing knowledge, clinical and technical competence, sound clinical judgment, professionalism as it relates to interpersonal and general communication skills, timely and complete documentation and take responsibility for ongoing professional development and competency validation. The APP role in partnership with supervising/collaborating physician(s) s includes history taking, clinical assessment, and physical examination, and order entry, interpretation of diagnostic studies, clinical diagnosis, treatment, and evaluation of care for the given population. The APP may have On Call duties as assigned by practice. APP will participate in inpatient weekend rotation. Performs other duties as assigned .
