As a MAT Nurse Practitioner , you will work alongside correctional officers and other medical professionals to provide support to the patients within the facility struggling with substance abuse. Manage induction, stabilization, maintenance, and tapering phases of MAT as clinically indicated. Collaborate closely with correctional healthcare staff, custody staff, mental health providers, and substance use counselors to ensure coordinated, safe, and effective care. Monitor patient adherence, response to treatment, and potential diversion risks within a correctional setting. Provide patient education on OUD, MAT, relapse prevention, overdose risk, and continuity of care upon release. Participate collaboratively with an interdisciplinary team to create individualized treatment plans that address medical, behavioral health, and social determinants of health. Ensure accurate, timely, and compliant documentation in the electronic health record, including MAT-specific reporting and regulatory requirements. Participate in discharge planning and continuity-of-care coordination, including referrals to community-based MAT providers upon release. Maintain compliance with state, federal, and facility regulations related to controlled substances, MAT delivery, and correctional healthcare standards.
