Registered Nurse (RN) House Supervisor - Full Time - Rappahannock General Hospital. Rappahannock General Hospital is a 25 bed not-for-profit Critical Access Hospital (CAH) that provides acute inpatient services located in Lancaster County. The Hospital provides 24-hour emergency care, as well as diagnostic and treatment services on an outpatient basis. Job Summary: Supervises the clinical and administrative functions of the Hospital through proven management skills during designated tour of duty. Provides leadership and direction to nursing staff, collaborates with physicians and other departments throughout the Hospital, and the community in problem solving and resolution. Combines clinical skills, and organizational and administrative abilities to provide quality patient care through innovation. Essential Functions: Performs hospital rounds to assess and evaluate the quality of patient care and to identify problems and needs in all areas of the facility; implements appropriate follow-up action. Serves as a clinical resource and support to nursing staff by performing patient assessment, inserting IV catheter, NG tube insertion and other patient related treatments and assistance when necessary. Provides assistance to staff in the use of monitoring equipment, IV pumps and other such equipment used in patient treatment. Acts as a resource person if the unit/division is unable to solve a staffing dilemma. Assists staff in utilizing staff to improve productivity and reduce agency usage. Assigns bed placement for emergency admissions, direct admissions and transfers, considering patient comfort and need, unit staffing and workload. Obtains patient status from attending physician on direct admits Initiates appropriate counseling and disciplinary action of employees in the Nurse Manager’s absence in accordance with Human Resources and the Department of Nursing policies; this includes following policy regarding substance abuse. Coordinates and performs patient transport form one area to another to expedite patient movement when needed. Utilizes resources to maintain patient flow and avoid diversion Responds immediately to all codes, Code Atlas and Code Red to assist assess needs and take appropriate actions according to policy. Initiates snow and disaster plans when necessary. Reviews and signs EMTALA forms to assure transfers are documented correctly. Resource for Care Management. Assists with resolving issues of nursing home placement, transportation, adoption and DME on evenings, nights and weekends. Assists staff with accurately reflecting discharge disposition in computer. Communicates with and assists supervisors in Housekeeping with patient flow and housekeeping issues. Priorities bed cleaning in a coordinated effort with the EVS supervisor. Assists as needed to coordinate ancillary services to provide procedures for patients, i.e., Cath Lab, OR, CT, etc. Procures equipment, supplies and patient nourishment in the absence of ancillary personnel. Utilizes Agility tracking for equipment needs. Transports remains to morgue and replaces the code cart. Resolves complaints as expediently as possible, communicating incident and resolution to appropriate management and/or notifying the administrator on-call. Acts as Patient Advocate on off shifts. Utilizes Risk Management as resource in resolving patient care issues. Provides review of protocols for Blood, TB and Flu exposure. Functions in place of Employee Wellness and Infection Control RN on off shifts. Utilizes a problem solving approach when dealing with conflict resolution between staff members, medical staff, visitors or other hospital employees. Serves as administrative resource for staff, physicians, patients, and families in accordance with Hospital policies and procedures. Provides support to families in conjunction with Pastoral Care and Patient Advocates. Notifies appropriate person of unusual hospital occurrence; e.g., patient’s concerns, personnel conflict and any other emergency situations. Demonstrates the knowledge and skills necessary to provide care appropriate to the age and needs of the patients served on his or her assigned unit. Demonstrates knowledge of the principles of growth and development of the life span and possess the ability to assess data reflective of the patient’s status and interpret the appropriate information needed to identify each patient’s requirements relative to his or her age, specific needs and to provide the care needed as described in the departmental policies and procedures. Provides Patient Care based on specifics age group in assigned unit: Employment Qualifications Must be a graduate from an accredited school of nursing. BSN preferred. Must be licensed and currently registered as a professional nurse in the State of Virginia. 3-5 years recent clinical experience including work experience in a supervisory role. Must be CPR certified and be able to perform both adult and child CPR in any setting throughout the hospital. Assessment. 1. Compiles database and formulates nursing diagnosis. 2. Utilizes physical assessment and interviewing techniques. 3. Anticipates need for intervention by other disciplines. 4. Determines priorities for patient care needs or problems. 5. Reassesses the patient as indicated by changes in condition and/ or response to treatment(s) B. Planning. 1. Initiates and updates a written nursing plan of care for patients by incorporating scientific and psychosocial principles that support the nursing diagnosis. 2. Incorporates information obtained from nursing care planning conferences into written nursing plan of care. 3. Coordinates patient, family, significant other, and health team members in the planning of hospital and post-discharge care. C. Implementation. 1. Performs clinical nursing skills independently. 2. Administers medications, IV therapy and treatments appropriately and accurately. 3. Initiates appropriate emergency procedures as necessary. 4. Appropriately delegates and coordinates the duties of ancillary personnel. 5. Communicates the patient's status and plan of care through written documentation in the patient's medical record. 6. Verbally communicates the patient's status to physicians, nurses and other health care professionals. 7. Communicates in a therapeutic manner with the patient, family and visitors. 8. Applies teaching/learning principles in education of patient and family. D. Patient Care Coordination 1. Manages daily patient assignment by appropriately setting priorities. 2. Delegates responsibility appropriately and monitors the nursing care delivered to patients by ancillary personnel. 3. Coordinates patient activities with other disciplines. E. Teaching and Quality Improvement 1. Develops, documents and implements Patient Teaching and Discharge Planning Record. 2. Participates in established patient, family, staff and/or student education program. 3. Participates in clinical monitoring at the unit level. 4. Identifies unit, staff and patient care problems. Takes action or refers problems to appropriate resources when indicated. F. Professional Excellence Model 1. May participate in activities of the RN 3 with the exception of serving as a facilitator for the Nurse Residency Program 2. If competent, serve as a charge RN and monitor productivity/patient flow 3.
