Job Title: Registered Nurse Department : Cardiac Stroke Unit Location : Unity Hospital Hours Per Week : Full Time - 36 hours Schedule : Nights 7p-7a; Every other weekend and holiday. The Cardiac Stroke Center – 2300 The Cardiac Unit is an 18 bed cardiac nursing unit that provides care to cardiac patients and provides full telemetry monitoring. This unit is focused on caring for the cardiac patient after cardiac procedures such as Pacemaker Insertion, Angiograms, Cardiac Ablations, Angioplasty, and patients who require medicated cardiac infusions. Our team of seasoned and experienced nurses provides mentorship and support to new staff, working together to drive critical thinking through complex medical situations. Many of our nurses start with us and transfer to higher acuity areas, feeling confident they have gained a solid foundation. For those that stay and continue their career with us, we help them sharpen and hone their nursing skills, always offering continuing education and support. CLINICAL LADDER LEVEL I-IV The RN level will be determined at the time of hire based on the candidate’s qualifications and ability to meet Clinical Ladder requirements. An established compensation add-on is applied to each RN level. Level expectations include: RN Level II: Minimum 6 months RN experience; AAS in Nursing; completion of Clinical Ladder requirements within 12 months, including 3 activities per pillar (Leadership, Quality, Organizational Contribution), 9 CEUs, and 6 flex items. RN Level III: Minimum 1 year RN experience; AAS in Nursing; demonstrated clinical and leadership skills; preceptor and/or charge experience; completion of 4 activities per pillar, 12 CEUs, and 15 flex items. RN Level IV: Minimum 1 year experience in the specialty hired into; BS in Nursing required; expanded leadership role (charge, preceptor, mentor); completion of Clinical Ladder requirements within 12 months, including 5 activities per pillar, 15 CEUs, and 20 flex items. RESPONSIBILITIES : Provides high-quality, patient-centered, clinical care Performs initial and ongoing assessment of the patient, utilizing appropriate data (i.e. physical, psychological, educational and risk-screen data appropriate for the chronological / developmental age of the patient). Communicates and documents assessment of patient with the interdisciplinary team. Utilizes all available resources in order to meet the needs of the patient and family. Develops and documents plan of care, treatment and services which is individualized and appropriate for the patient needs, strengths, limitations and goals. Complies with organizational policies, procedures and protocols including policies governing safety, medication administration and documentation. Revises plan of care based on ongoing evaluations and updates documentation appropriately. Completes at least one clinical research utilization (CRU) form for submission to Nursing Research Department utilizing a research article from the last 5 years from research performed in the United States. Compliance Completes a minimum of 5 nationally approved contact hours (CNE or CEU) per year. Completes mandatory in-services and health screens in timely manner. Maintains Basic Life Support (BLS) certification. REQUIRED QUALIFICATIONS: Diploma or Associate’s Degree in Nursing Registered Nurse license in New York State. Basic Life Support (BLS) certification. Job Description The Heart Failure Navigator is a specialized clinical professional responsible for coordinating and optimizing care for patients with heart failure across the continuum—from inpatient care through discharge and outpatient management. The role focuses on improving clinical outcomes, reducing readmissions, achieving LOS targets, and ensuring adherence to evidence-based heart failure guidelines. The Heart Failure Navigator facilitates seamless transitions across the continuum of care, reduces readmissions, improves compliance with guideline-directed therapy, and enhances the patient experience through education and coordinated follow-up. Patient Care Coordination Identify and follows heart failure patients during inpatient admission and outpatient encounters Coordinate multidisciplinary care (cardiology, primary care, nursing, case management, pharmacy, rehab) Facilitate smooth transitions of care from hospital to home, rehab, or outpatient follow-up Ensures post hospital discharge appointments are in place prior to discharge Clinical Management & Monitoring Ensure adherence to evidence-based clinical guidelines (e.g., GDMT for heart failure) Monitor patients for clinical deterioration and intervene early Collaborate with providers to optimize medical therapy Review labs, imaging, and diagnostics relevant to heart failure management Patient & Family Education Provide education on: Disease process and symptom recognition Medication adherence Dietary restrictions (e.g., low sodium) Daily weight monitoring and self-management Reinforce discharge instructions and self-care plans Readmission Reduction & Outcomes Management Develop and implement strategies to reduce 30-day readmissions Conduct post-discharge follow-up calls or visits Track and report quality metrics (LOS, readmissions, mortality, compliance) Monitors LOS and develops strategies to help achieve targets Care Transition & Discharge Planning Ensure timely follow-up appointments (cardiology and primary care) Coordinate discharge needs (home care, equipment, medications) Address barriers to care such as transportation or medication access Quality Improvement & Program Development Participate in heart failure program development and accreditation initiatives Analyze data to identify trends and opportunities for improvement Support regulatory compliance (e.g., Joint Commission, CMS metrics) Documentation & Reporting Maintain accurate documentation in the electronic medical record Track patient outcomes and registry data Prepare reports for leadership and quality committees Required QUALIFICATIONS Bachelor's in Nursing or related field. Valid State or national Nursing License. 2 years of experience in Acute Care or relevant area.
