Arranges discharge medical equipment, supplies, and medications and arranges or provides related training as indicated. Maintains Interdisciplinary Rounding List for discharge planning. Conducts discharge follow up calls for identified patients/families. Minimum Qualifications EDUCATION: Bachelor's degree in nursing or associate degree in nursing. EXPERIENCE: Two (2) years of nursing or case management experience. For NICU – Three (3) years of NICU nursing experience. LICENSURE OR CERTIFICATION: Current RN licensure permitting work in state of Arizona and basic life support (BLS) required.
