Enliven is seeking an experienced RN (Registered Nurse) to service home infusion and Infusion Center patients in Lufkin and surrounding locations. The ideal candidate must be well-trained and able to give the best nursing care possible with little supervision. They will educate patients and their families on prevention and healthy habits. The ideal candidate must be compassionate, cool-tempered, and capable of handling emergent situations. They must follow health and safety guidelines faithfully and consistently. The goal is to promote patients' well-being by providing high-quality nursing care. Responsibilities: RN (Registered Nurse) Complete daily electronic forms for each assigned patient within a secure Electronic Medical Record (EMR) System. Provide admission, case management, and follow-up skilled nursing visits for home infusion and infusion center patients. Administer ongoing care/infusions for each patient, and provide necessary follow-up as directed by the Physician and Clinical Manager. Confer with physicians and pharmacies to develop the initial treatment plan based on the physician's orders and the initial patient assessment. Provide hands-on care, management, and evaluation of the care plan and teaching of the patient under a physician based on ongoing assessments and as required by policy/regulation. Report patient care, conditions, and progress to the patient's physician, pharmacy, and Clinical Manager continuously. Implement patient care plan in conjunction with patient and family to assist them in achieving optimal resolution of needs/problems. Discharge patients after consultation with the physician and Clinical Manager, preparing and completing needed clinical documentation. Prepare appropriate medical documentation on all patients, including case conferences, patient contacts, medication order changes, re-certifications, progress updates, and care plan changes. Prepare visit/shift reports, update/summarize patient records, and confer with other healthcare disciplines to provide optimal patient care. The RN will evaluate and monitor the health and safety needs of individuals with developmental disabilities, promote wellness, and ensure nursing care is delivered in compliance with applicable federal and state regulations and established policies. · All schedules are flexible. Essential Responsibilities · Coordinate medical care for assigned individuals; monitor and assess health needs and arrange follow-up care as needed. · Assess health status through direct observation, including signs and symptoms of discomfort, distress, or illness; evaluate physical status changes and responses to care and treatment; document and follow up on findings. · Communicate with physicians, families, interdisciplinary team members, and clinicians involved in providing services. · Communicate and follow up on hospitalizations, emergency room visits, injuries, trauma, accidents, illness, distress, and other health concerns as required. · Accompany individuals to medical appointments when necessary to advocate for and discuss health issues and communicate findings to team members. · Respond, when necessary, to urgent or emergent calls outside routine working hours regarding individuals assigned to the caseload. · Attend meetings with management, staff, families, and clinicians regarding health-related issues. · Develop individual-specific nursing plans and protocols and update them as an individual's status changes. · Supervise AMAP administration of prescribed medications and routine treatments in accordance with federal and state regulations and established policy. · Administer STAT medications and treatments as prescribed; participate in in-services and train AMAPs as needed; complete AMAP certification, recertification, and decertification processes as necessary. · Perform other duties as assigned. Qualifications · Must be currently licensed as a Registered Professional Nurse (RN) in New York State. · CPR certification required. · Experience working with individuals with developmental disabilities preferred. · Must demonstrate excellent written and oral communication skills. · Ability to handle multiple priorities and work as a member of a team. · A valid NY State or Tri-State Driver’s License for more than one year, with a good driving record, is preferred.
