Utilization Review Nurse-RN - Main Case Management - Full Time - Days

Job Description To maintain high-quality, medically necessary, evidence-based care, and efficient treatment of all patients, regardless of payment source, by ensuring the patients receive the right care, at the right time, in the right place. Case Management Model: utilize an Integrated Case Management Model. Under this model the Case Managers will follow patients through the continuum while facilitating the functions of utilization review, utilization management, and cost containment. Track and trend denials and payor issues to provide feedback and education to payer relations and the case management department. Responsibilities Clinical review of 100% acute bedded patients admitted to Inpatient or Observation status at The Christ Hospital against medical necessity criteria (Interqual and MCG) for appropriateness of admission. Demonstrate understanding of evidenced based medical necessity criteria. Maintains efficient methods of ensuring the medical necessity and appropriateness of all hospital admissions. Identify/facilitate patient status from observation to inpatient as patient clinical condition warrants. Compliance with all Medicare regulatory requirements Work with external payers completing/securing authorization for all services provided. Monitors cases for appropriateness of continued stay, level of care and services, and quality of care using approved screening criteria. Communicate with physicians when alternatives to inpatient care are indicated by clinical review. Identify cases needed for second level of review- refers cases to the Physician Advisor that do not meet established guidelines for admission or continued stay. Consistent collaboration with the RN Case Manager to prevent extended length of stays and appropriate status determination. Identifies potential delays in service or treatment and refers to the appropriate individuals within the multidisciplinary patient care teams for action/resolution. Track and trends avoidable day information in Midas per process. Identifies problems related to the quality of patient care and refers such problems to the Performance Improvement Department. Adherence to department productivity standards. Initial, concurrent, and retro reviews should be completed timely including all necessary information for approval of claims. All reviews should contain information only pertinent to IS/SI (Intensity of Service/Severity of Illness). Compliance with documentation methods for monthly reporting and statistics for presentation to the Utilization Review Committee. Interfaces with patient registration and patient financial services etc. to collaborate on financial issues. Establish an effective rapport and relationship with third party payers to promote cost effective clinical outcomes. Strong presentation, written and oral communication skills, with strong analytical and problem-solving skills as well as time/project management skills. Ability to work with a variety of disciplines and levels of staff across departments and the organization is required. Administers medications and treatments as prescribed by the physician in the physician’s plan of care. 8. Counsels the patient and family in meeting nursing and related needs. 9. Reports must be submitted immediately when the reporting person has reasonable cause to believe that abandonment, abuse, financial exploitation, or neglect of a vulnerable adult has occurred. 6. Teaches the patient and family/caregiver self-care techniques as appropriate. Provides medication, diet and other instructions as ordered by the physician and recognizes and utilizes opportunities for health counseling with patients and families/caregivers. Works in concert with the interdisciplinary group. 7. Provides and maintains a safe environment for the patient. 8. Assists the patient and family/caregiver and other team members in providing continuity of care. 9. Works in cooperation with the family/caregiver and hospice interdisciplinary group to meet the emotional needs of the patient and family/caregiver. 10. Attends interdisciplinary group meetings. Supervises ancillary personnel and delegates responsibilities when required. 4. Scope Of Position The professional nurse practices, delegates, and collaborates with other team members to facilitate the coordination and delivery of patient care for the assigned patient population. The patient population identified as neonate, adolescent and adult. Reports to Director FBC. Responsibilities Nursing Process - Provides quality of care utilizing the nursing process. Develops plan of care with appropriate intervention for the ante partum, intra partum, post-partum and newborn patients. Supports Breast Feeding initiatives. Leadership - Demonstrates leadership to ensure all patient/family needs are met. Delegates responsibilities to appropriate member of the care team. Has the autonomy capability and motivation to be identified as one of principle care givers for the patient providing appropriate teaching and communicating pertinent information across continuum of patient care. Directs accesses and provides patient care to outpatients as well as inpatients. Professional Behavior - Acts as a role model by promoting a professional and collaborating approach with all members of the care team collaborates and works cooperatively to resolve problems and ensure excellence in clinical practice. This may include acting as a mentor and/or preceptor. Demonstrates and promotes cultural sensitivity. Quality - Actively participates in initiatives to ensure quality outcomes of patient care. Adheres to and responsible for meeting all standards of care as it relates to AWHONN, American Academy of Pediatrics, American College of OB, GYN, JCAHO, national and local regulatory requirements. Competency - Maintains accountability for professional competency. Demonstrates competent knowledge and critical thinking to promote optimal patient care in the FBC including Surgical Rooms. Maintains required knowledge and skills, completes general unit/department specific competencies. Requirements Graduate of an accredited RN program State of CT licensed or license eligible RN BLS/ACLS required Neonatal Resuscitation Certification required EFM Certification - To be obtained within eighteen (18) months of hire Must complete AWOHNN classes - Intermediate then Advanced Fetal Monitoring Classes every two (2) years Must possess excellent written and verbal communication skills Ability to work in a time-oriented environment including exposure to all patient elements in blood borne pathogenics, physical requirement lifting and pulling patients, pushing stretches, standing and walking. May be exposed to environmental hazards such as anesthesia gases or elements. Ability to interact with patient and families with tact and confidentiality.
