Join UChicago Medicine, as an Oncology Nurse Navigator in the HHT and Lymphoma at our Hyde Park Campus. The Oncology Nurse Navigator directs the coordination of care for patients with Hereditary Hemorrhagic Telangiectasia (HHT) and Lymphoma patients. As an Oncology Nurse Navigator (RN) for the HHT and Lymphoma Teams, you serve as clinical liaison and the single point of contact for patients, coordinating their care as they move from initial diagnosis, treatment, and survivorship. You help ensure timely delivery of care and eliminate potential barriers, identify resources, and serve to enhance patient care and the patient experience. In this role, the Oncology Nurse Navigator is responsible for the coordination of all aspects of patient care for this population including education, multidisciplinary referrals, data tracking, diagnostics, determining protocol eligibility, treatment orders, consents and other activities essential to the overall care of the patient. Participates in the conduct of clinical research trials including multi-center cooperative group and intergroup trials, multi-institutional pharmaceutical trials (Phase I, Phase II/III trials), and in-house, Investigator-Initiated trials designed, implemented and coordinated in the Hematology/Oncology Section. Serves as liaison between Attending Physicians, Advanced Practice Providers, Primary Investigators, sponsors, patient and family members, clinical research coordinators, referring physicians and community providers, pharmacists, social workers, financial specialists and other members of the multidisciplinary team. Directs all patient care needs and required care activities to ensure smooth care. Organizes all diagnostic workup. Responsible for scheduling admissions and preparing the appropriate paperwork to facilitate a clear and concise admission process. Throughout course of treatment, documents care appropriately and in accordance with all local, state and federal guidelines. Coordinates all follow-up care and diagnostics per standard operating procedures. Communicate with all members of the multidisciplinary team, patients and caregivers throughout the treatment process. 20% -- Data Collection and Interpretation. Evaluate all lab results for signs of toxicity or complications related to either the treatment or disease. Review all radiology and other test results for disease assessment and follow response criteria according to the research protocol and/or standard of care. Report adverse drug reactions and submit the appropriate documentation as directed in the research protocol. Follow-up on lab tests specified by the study and additional tests at the discretion of the physician to evaluate toxicities and treatment. 20% -- Education Activities. Educates patients and their family regarding clinical aspects of their disease, purpose and nature of treatment, research protocols including potential impact on the patient should they agree to participate (i.e., treatment schema, tests, clinic visits, etc.), transplant process, calendar of events, symptom management and follow-up care post-transplant. Teach patients and their family members on injections, signs and symptoms, and medication side effects. Coordinates and conducts patient and family education utilizing NCODA or HHT Cares resources. Participate in the development of educational resources and materials for patient and family teaching. 10% --Process Improvement. Participate in process improvement projects to address lymphoma or HHT operations. Accountable to actively participate in task forces and meetings dedicated to improving communication among team members. Participates in policy and procedure reviews and provides feedback. Accountable for adhering to program policy and procedures and other regulatory agencies. Required Qualifications Must have a Bachelor’s degree in Nursing from an accredited academic body. Must be a licensed Registered Nurse with a valid, unrestricted Illinois State license. Must have a minimum two years of clinical experience working as an RN in Hematology or Oncology Specialty. Must have demonstrated clinical competence in patient care, teaching and management of patients with a cancer diagnosis, and knowledge about clinical aspects of cancer and its prevention, diagnosis, and treatment. Job Details St. Francis Hospital Critical Care Advanced Practice Provider (APP) is a valuable team member. Primary responsibility is the management of critically ill and complex (multi-organ) patients in an intensive care setting. Job Summary Provides clinical direction for all aspects of patient care in an acute inpatient setting, fostering patient/family centered care. Demonstrates clinical competency utilizing evidence-based practices/research in all areas of the nursing process: assessment, planning, implementation, evaluation and outcomes. Incorporates knowledge of safety protocols, cost containment and quality improvement throughout the nursing process. Identifies patient, unit and organizational priorities and incorporates into the nursing process. Adheres to all compliance and policy/procedures of the organization including, but not limited to licensure renewal, assigned training, employee health screenings, time and attendance policy, dress code policy, patient confidentiality, infection control, medication administration. Demonstrates knowledge of regulatory agency requirements (TJC, SC DPH, CMS). Provides care within the scope of the state Nurse Practice Act. The acute inpatient setting may require staff to be scheduled for a weekend, holiday, and on-call work. Collects data and completes nursing assessment utilizing appropriate area and/or age specific standards/policies and tools. Completes appropriate forms. Assesses and recognizes subtle or actual changes in patients' status and patient response to intervention. Identifies nursing priorities for patient plan of care based on nursing assessment and other data as appropriate. Validates these priorities with healthcare team members and documents appropriately. Develops an individual plan of care based on the age and assessed needs of the patient. Develops the plan in collaboration with the patient and/or family, the provider, and other members of the interdisciplinary health care team. Updates the plan based on changes in the patient's condition and communicates changes in the plan to other members of the team. Identifies realistic and measurable goals and outcomes to be attained prior to discharge. Validates these with the patient and/or family. Demonstrates respect for the patient's/family cultural, spiritual, and belief system in planning for care. Includes planning for discharge to assure continuity of care and a smooth transition post discharge from one level of care to another including appropriate referrals. Provides direct care to assigned patients consistent with the established nursing plan. Performs specific tasks based on organizational, departmental and/or unit policies and procedures within the RN scope of practice. Delegates nursing tasks as appropriate to members of the healthcare team, developing written assignments (if appropriate) and ensures completion of assigned tasks. Monitors quality of care delivered on a continual basis; ensures appropriate surveillance of assigned patients. Provides care based on patient and unit priorities, coordinating and organizing tasks as appropriate, utilizing resources internal and external to the unit. Incorporates cost containment principles into nursing care. Utilizes appropriate technology in daily practice. Assumes responsibility for patient safety by maintaining a safe environment to include awareness of safety procedures, protocols and adherence to patient care standards. Utilizes performance indicators to collect data, monitor quality and effectiveness while providing nursing care. Completes assigned hospital/unit performance improvement monitoring (audits). Based on assessment/reassessment of patient response to interventions, evaluates plan of care for effectiveness, documents progress toward expected outcomes, and revises plan of care as needed. Reports changes in patient condition and refers unresolved problems to appropriate healthcare team members. Discusses outcomes with peers (via shift-to-shift report) and with other healthcare team members. Evaluates opportunities for quality improvement; based upon evaluation, incorporates performance improvement recommendations into the plan of care as appropriate. Identifies/assesses patient/family learning needs and teaches accordingly by using language and terminology that the patient/family is familiar with providing examples to clarify ideas, implementing and evaluating teaching plan (encourages patient/family involvement/participation), Computer resources and Micromedex utilization Communicates the patient/family's need for education/teaching to others in the healthcare continuum. Evaluates own nursing practice in relation to professional practice standard guidelines, state statutes, policies, procedures, rules/regulations and current specific knowledge. Takes personal accountability for maintaining competency through education and professional activities. Maintains professional licensure and completes required mandatory education in a timely manner. Reflects on identified areas of strength that offer opportunity for improvement. Ensures that written documentation and oral communication focuses on pertinent information to support meeting patient/family care needs. Serves as the patient's advocate and coordinator of care, working collaboratively with providers and other members of the healthcare team. Performs other duties as assigned. Supervisory/Management Responsibility This is a non-management job that will report to a supervisor, manager, director or executive. Minimum Requirements Education - Associate degree in Nursing. Bachelor's degree in Nursing preferred. Experience - One (1) year experience as a registered nurse. ​ In Lieu Of In lieu of one (1) year of registered nurse experience, will accept successful completion of the Prisma Health Nurse Residency Program or successful completion of a comparable residency from another organization. A Fred Hutchinson Cancer Center ARNP/PA-C is a certified Physician Assistant or Advanced Registered Nurse Practitioner who provides evaluation and medical management for Hematology and Hematologic Malignancy patients following bone marrow transplantation, specializing in the management of graft-versus-host disease and other post-transplant complications. This role offers the opportunity to care for a unique patient population facing health and life challenges, combining comprehensive primary care responsibilities with highly sub-specialized, complex care. The ARNP/PA-C follows patients, provides supportive medical care, and conducts surveillance after treatment completion while working collaboratively with a multidisciplinary team to coordinate and optimize patient care. There are also opportunities to participate in clinical research studies. All clinical responsibilities are performed within the scope of practice, institutional privileges, and under the supervision of an attending physician. Responsibilities Performs complex and problem-focused H & P’s Evaluates and manages on-treatment and post-treatment side effects, as well as long-term surveillance and survivorship issues. Performs procedures (bone marrow aspiration and biopsy, lumbar puncture with intrathecal chemotherapy, etc.) according to licensure, credentialing, and scope of practice Collaborates and consults with other disciplines and services to ensure safe, effective, and timely treatment as well as continuity of patient care.
