As a Hospice RN – Regional Support, You Will: Deliver high-quality, compassionate nursing care for hospice patients Manage and implement individualized plans of care Support and lead interdisciplinary team members (LPNs, CNAs, HHAs, etc.) Assess and address patients’ physical, emotional, spiritual, and environmental needs Accurately document all care provided and ensure compliance with hospice protocols Educate families and caregivers on care instructions, medications, and safety Participate in the on-call rotation and provide after-hours care as needed Collaborate with physicians, referral sources, and the care team Assist with referrals, regulatory documentation, and payer compliance Travel to various Gentiva hospice locations as assigned to fill urgent needs Schedule & Travel: This role typically follows a Monday–Friday schedule, with occasional on-call responsibilities. Travel is required and may include: Option 1: Overnight travel required to cities within a 100-mile radius. Option 2: Frequent overnight travel to multiple states is required. Option 3: Regional travel with overnight stays based on assignment needs. A company fleet car is provided for eligible employees.
