Maintains appropriate boundaries. Receives and processes referral information from Intake. Explains hospice care and Keystone Hospice services and/or facility services and programs to prospective patients/families, physicians, and other appropriate individuals. Consults with the Medical Director, attending physician, and/or mid-level provider (if any) on patient’s eligibility for hospice care. Completes all required admission paperwork (e.g. admission packet) and consents for care with patients/families timely and accurately. Conducts and documents completion of initial assessments accurately and within required time frames. Provides or collaborates with IDT to provide care needed to address patient/family immediate needs, as identified in the initial assessment. Conducts and documents start of/completion of comprehensive assessments accurately and within required time frames for all assigned patients. Coordinates/communicates necessary information with other members of the IDT, consulting physician(s), facility staff, and others as appropriate. Participates in development of the Plan of Care, as appropriate, for each assigned patient. Serves as a resource for all Hospice and/or facility staff to facilitate continuity of patient care and enhance the quality of patient care. Obtains and documents physician orders, as necessary. QUALIFICATIONS/REQUIREMENTS: Hospice & Long-Term Care Registered Nurse EDUCATION & LICENSURE BACKGROUND Registered Nurse with current license to practice in Indiana. Bachelor’s degree in nursing preferred, but not required. Minimum of one (1) year clinical nursing experience; hospice experience preferred, but not required. Demonstrated knowledge of the regulatory requirements for hospice. DEGREE OF TRAVEL Travel between long-term care facilities and/or place of primary residence. Attend bi-weekly IDT meetings, if requested. *Candidates must be fully Vaccinated against Covid 19, or obtain a religious or medical exemption upon hire.
