Overview This position is fully in the ambulatory or outpatient setting, with no inpatient responsibilities. The clinician will play an essential role on a multidisciplinary team, assisting in the delivery of care for patients undergoing care transitions (e.g., from the emergency department or inpatient stay to home), managing follow-up for chronic diseases, and closing health maintenance gaps. The role involves providing comprehensive diagnostic and therapeutic health care services to patients, including but not limited to: taking medical histories, performing medication reconciliation, conducting physical examinations, ordering and coordinating lab tests and diagnostic studies, prescribing medications, and guiding patients on treatment plans and health education. Specific responsibilities include conducting post-discharge or transitional care management visits, Medicare Annual Wellness Visits, and chronic disease management visits. This work is conducted under the direction of UFHP and ambulatory care leadership and will interface with interdisciplinary teams across the health system. Patient interactions will primarily occur through telemedicine visits and telephone outreach (with the potential to see patients in-person one day per week), requiring strong electronic medical record (EMR) proficiency and excellent telemedicine communication skills with both patients and team members. Qualifications Minimum Education and Experience Requirements: Master's degree in Nursing or Physician Assistant. Credentialing: Must meet requirements set forth by the Medical Staff Office to obtain and retain privileges to practice as an Advanced Practice Registered Nurse or Physician Assistant within UF Health Shands Hospital and/or the Shands UF Healthcare Network, as applicable. Essential Functions and Responsibilities : Provide patient care on an outpatient basis to include procedures normally performed in an urgent care setting Responsible for the delivery of care, treatment, and referrals. Arrange for referral resources for those medical specialties not provided by the organization for patients of the clinic Evaluate patients to determine the medical condition of the patient, through the performance of physical examinations, thereby, establishing a diagnosis and proposed treatment plan Perform such clinical procedures for patient's benefit which are within scope and skill of practitioner Communicate information to patients regarding diagnosis and treatment goals Maintains accurate , complete, concise, and timely documentation with a focus on proper level coding and clinical documentation integrity (CDI) in the electronic medical record Ability to perform occupational and telemedicine patient care Perform non-patient facing duties such as lab and x-ray review Perform other job-related duties as may be assigned by the Medical Director What’s Required: Current, unrestricted PA or NP license in the state or the ability to obtain Ability to obtain : DOT certification within 60 days of hire DEA licensure Knowledge of treatment and drug interactions Excellent communication and interpersonal skills Compensation: Pay is based on experience, skills and falls within an established range. The Weight Management APC will evaluate and treat patients in the medical weight loss program in a high-volume office with a large patient panel. This role will also work in partnership with our bariatric surgery team to provide postop bariatric surgery care as needed.
