The RNCM provides case management, clinical care and is responsible for coordination all disciplines involved in providing quality, cost-effective and billable care through direct care and supervision of care under the direction of the physician. The RN educates patients and their family members and ensures the safety of the patient. The RN is accountable for completing accurate documentation and remains compliant with all legal rules and regulations. The RN is responsible for all practices and duties within the scope of practice as outlined by the state. KEY RESPONSIBILITIES Oversees the overall care of the patient from beginning to end of episode Updates all doctor orders in patient chart accordingly Notifies doctor of any significant changes of the patient’s condition Manages multi-disciplinary care as applicable Provides skilled nursing care in a patient’s home as ordered by the attending physician Observes and monitors patient conditions Performs OASIS assessments to develop an individualized plan of care and makes adjustments as needs change Administers medication as prescribed by the physician Helps decrease re-hospitalizations by front loading visits for high risk patients Promotes continuity of care with appropriate admissions, transfers and discharges Counsels patient and family on the disease/injury processes and how to manage Incorporates patient and family in development of plan of care Helps decrease re-hospitalizations by teaching the patient on preventative measures and making good decisions Listens to patient and family members Oversees and supervises total care of patient provided by nurse aides and LPNs Evaluates the treatments and medical condition of the patient according to the plan of care Determines if the level of care being provided meets the patients’ needs Communicates with the LPNs and nurse aides on supervisory visit results Completes all clinical documentation in accordance with agency protocol and Medicare/Federal guidelines Documents all aspects of treatment, assessments, and patient education Maintains active RN license Communicates with scheduler any changes outside the normal Participates in all on-call requirements and case conferences Attends in-service trainings and mandatory agency meetings Drives to patient’s primary location per scheduled visit. Daily attendance at assigned visit locations. Documents all aspects of subsequent, discharge, eval/recert/resumption of care visits within 24 hours of visits. Documents all aspects of start of care visits within 24 hours of visits. Completes and submits all required documentation within specified company requirements. Follows plan of care as permitted within the scope of practice for a Registered Nurse. Fulfills RN visits in assigned geographic location per patient need within federal and state specifications. Other duties as assigned. QUALIFICATIONS Minimum of one year of experience as an RN in an acute care setting. Home care experience is desired. Ability to effectively communicate and create positive impressions with patients, families, physicians and co-workers. Ability to remain calm, have patience and be accommodating. Compassionate and caring while working with patients. Knowledgeable on nursing best practices. Ability to make appropriate nursing judgments. Ability to identify a situation and handle it with the best possible solution. Detail-oriented and observant. SUMMARY: The primary purpose of the Assistant Director of Nursing is to assist the Director of Nursing in planning, organizing, developing, and directing the day-to-day functions of the Nursing Department in accordance with current federal, state, and local standards, guidelines, and regulations that govern our facility, and as may be directed by the Administrator, the Medical Director, and/or the Director of Nursing, to ensure that the highest degree of quality care is maintained at all times. ESSENTIAL DUTIES AND RESPONSIBILITIES: · Assist the Director of Nursing in planning, developing, organizing, implementing, evaluating, and directing the day-to-day functions of the nursing department, in accordance with current rules, regulations, and guidelines that govern the long-term care facility. · Participate in developing, maintaining, and updating our written policies and procedures that govern the day-to-day functions of nursing department. · Ensure that reference material maintained at the nurse’s station is current. Recommend to the Director written material that will assist the nursing department in meeting the day-to-day needs of residents. · Develop and periodically update nursing service objectives and statements of philosophy. · Assist in developing and implementing a nursing organization structure. Make written and oral recommendations to the Director as necessary/required, concerning the operation of the nursing department. · Assist in developing methods for coordinating nursing services with other resident services to ensure the continuity of the residents’ total regimen of care. · Ensure that all nursing personnel are following their respective job descriptions. Recommend to the Director the number and level of nursing personnel to be employed. Assist the Director in the recruitment and selection of nursing personnel. · Ensure that a sufficient number of licensed practical and/or registered nurses are available for each shift to ensure that quality care is maintained. · Ensure that a sufficient number of Certified Nursing Assistants are available for each shift to ensure that routing nursing care is provided to meet the daily nursing care needs of each resident. Also ensure that all nursing assistants are enrolled in or have graduated from an approved nursing assistants program. · Develop work assignments and schedule duty hours, and/or assist nursing supervisory staff in completing and performing such tasks. · Delegate to the Charge Nurse and Nurse Supervisors the administrative authority, responsibility, and accountability necessary to perform their assigned duties. · Assist the Director in preparing performance evaluations as directed. · Make daily rounds of the nursing department to ensure that all nursing personnel are performing their work assignments in accordance with acceptable nursing standards. Report findings to Director. · Review complaints and grievances made or filed by department personnel. Make appropriate reports to Director as required or as may be necessary. · Ensure that departmental disciplinary action is administered fairly and without regard to race, color, creed, national origin, age, sex, religion, handicap, or marital status. · Interpret the department’s policies and procedures to personnel, residents, visitors and government agencies as necessary. · Ensure that all nursing personnel participate in the facility’s TB testing program. · Participate in the interviewing and selection of residents for admission to the facility. · Inform nursing personnel of new admissions, their expected time of arrival, room assignment, etc. Ensure that rooms are ready for new admissions. Greet newly admitted residents upon admission. Escort them to their rooms as necessary. · Review nurse’s notes to ensure that they are informative and descriptive of the nursing care being provided, that they reflect the resident’s response to the care, and that such care is provided in accordance with the resident’s wishes. · Schedule daily rounds to observe residents and to determine if nursing needs are being met. · Monitor medication passes and treatment schedules to ensure that medications are being administered as ordered and that treatments are provided as scheduled. Report medication errors to the Director. · Provide direct nursing care as necessary. · Report problem areas to Director. Assist in developing and implementing corrective action. · Participate in developing, planning, conducting, and scheduling in-service training that provide instructions on “how to do the job” and ensure a well-educated nursing department. · Participate in the mandatory new hire orientation. · Assist Director in scheduling annual mandatory in-service training for staff. (OSHA, HIPAA, Abuse Prevention, Infection Control, etc.) · Assist the Safety Officer in developing safety standards for the nursing department. · Assist the Director of Nursing in identifying, evaluating, and classifying routine and job-related functions to ensure that tasks in which there is potential exposure to blood/body fluids are properly identified and recorded. · Ensure that nursing personnel work areas (nurse’s station, medication rooms, etc.) are maintained in a clean and sanitary manner. · Ensure all resident care rooms, treatment areas, etc., are maintained in a clean, safe and sanitary manner. · Ensure that all nursing personnel participate in and conduct all fire safety and disaster preparedness drill in a safe and professional manner. · Ensure that an adequate stock level of medications, medical supplies, equipment, etc., is maintained on premises at all times to adequately meet the needs of the residents. · Assist the Maintenance Director in developing a preventive maintenance program for maintaining the nursing department’s equipment and supplies in a safe and operable manner. · Ensure that only trained and authorized personnel operate the department’s equipment in a safe manner. · Participate in the development of written preliminary and comprehensive assessments of the nursing needs of each resident. Assist the MDS/CPC in planning, scheduling, and revising the MDS, including the implementation of RAPs and Triggers. · Encourage the resident and his/her family to participate in the development and review of the resident’s plan of care. · Be sure that staff members are providing care that reflects the wishes of the resident. · Maintain confidentiality of all resident care information including protected health information. Report known or suspected incidents of unauthorized disclosure of such information. · Ensure all nursing personnel are knowledgeable of the residents’ responsibilities and rights include the right to refuse treatment. · Performs other duties as assigned. QUALIFICATIONS: To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. PURPOSE AND SCOPE: The professional registered nurse Outpatient RN CAP 1 is an entry level designation into the Clinical Advancement Program (CAP). This position is accountable and responsible for the provision and coordination of clinically competent care including assessment, planning, intervention and evaluation for an assigned group of patients. This may include delegation of appropriate tasks to direct patient care staff including but not limited to RNs, LVN/LPNs and Patient Care Technicians. PRINCIPAL DUTIES AND RESPONSIBILITIES: All duties and responsibilities are expected to be performed in accordance with Fresenius Kidney Care policy, procedures, standards of nursing practice, state and federal regulations. · Performs all essential functions under the direction of the Supervisor and with guidance from the Clinical Educator, Preceptor or in collaboration with other Registered Nurses. · Performs ongoing, systematic collection and analysis of patient data pre – during – post hemodialysis treatment for assigned patients and documents in the patient medical record, makes adjustments or modifications to treatment plan as indicated and notifies Team Leader, Charge Nurse, Supervisor or Physician as needed. · Assesses, collaborates, and documents patient/family’s basic learning needs to provide initial and ongoing education to patients and family. · Directs and provides, in collaboration with direct and ancillary patient care staff, all aspects of the daily provision of safe and effective delivery of hemodialysis therapy to assigned patients. · Administers medications as prescribed or in accordance with approved algorithm(s), and documents appropriate medical justification and effectiveness. · Initiates or assists with emergency response measures. · Serves as a resource for health care team, participates in staff training and orientation of new staff as assigned. · Ensures correct laboratory collection, processing and shipping procedures are performed and reschedules missed or insufficient laboratory collections. · Identifies expected outcomes, documents and updates the nursing assessment and plan of care for assigned patients through collaboration with the Interdisciplinary Team. · Ensures patient awareness related to transplant and treatment modality options. · Required to complete CAP requirements to advance. · Performs all other duties as assigned by Supervisor. EXPERIENCE AND REQUIRED SKILLS: · Entry level for RNs with less than 2 years of Nephrology Nursing experience as a Registered Nurse. · Chronic/acute hemodialysis experience (preferred).
