REGISTERED NURSE- PRN SUMMARY The Registered Nurse is responsible for providing nursing services, training, and support to address the medical or psychiatric needs of individuals with developmental disabilities, acquired brain injuries, substance abuse issues and/or emotional or behavioral challenges. ESSENTIAL JOB FUNCTIONS To perform this job successfully, an individual must be able to satisfactorily perform each essential function listed below: Conducts the medical assessment of each individual referred to the program and collaborates with an Intake staff member to determine appropriate placement in the community Participates in the development and implementation of service plans and supervises, in conjunction with the physicians, the medical components of the plan Reviews documents and revises all nursing/nursing related care plans on a regular basis Ensures medical records and reports are current and complete in accordance with regulatory and corporate standards, including medication administration records and physician’s orders Maintains contact with primary care physicians and specialists, and, when directed, attends doctor visits with mentor and/or individual receiving services. Promotes health and wellness for individuals and ensures nursing actions and steps are put in place for prevention Administers medications according to established guidelines Provides documentation for all medications given and reviews medication administration records for compliance Oversees the management of high-risk medical conditions and private duty nursing services Develops training flow charts specific to each individual receiving services, provides and documents medical skills training to clients, mentors, respite providers, staff, and where appropriate, the biological family Presents medical skills components at Pre-service Orientation Participates as an Individual Health Plan (IHP) or Plan of Care (POC) team member, Evaluates the medical needs of each individual for initial IHP/POC, Ensures IHP/POC goals and objectives are medically indicated, and Generates ongoing health care protocols for the specific medical conditions outlined in the IHP/POC Provides consultation and technical assistance to staff and mentors regarding home care issues along with operation of durable medical equipment. May be required to coordinate, organize and/or assist with household activities such as light housekeeping and meal preparation; transport individuals receiving services to planned and/or necessary activities and appointments Participates in on call system as needed May actively participate in IDT meetings, Human Rights Committee, Health and Safety Committee and Administration meetings Performs other related duties and activities as required SUPERVISORY RESPONSIBILITIES None required Minimum Knowledge and Skills required by the Job The requirements listed below are representative of the knowledge, skill, and/or abilities required to perform the job: Education and Experience: Graduate of an accredited RN program One year experience working with medically complex individuals. Work with residents, families, and other staff to ensure that residents are at appropriate levels of care. Manage labor costs, supplies, and other line items in clinical budget. Monitor documentation systems to ensure that all regulatory timeframes for review of assessments and service plans are met and that resident needs are met. Write and review residents’ assessments/evaluations and care-plans with appropriate staff. Assess/evaluate potential residents for Assisted Living level of care. This may involve assessing/evaluating potential residents in their home or another healthcare setting; must be able to drive to location and perform assessment/evaluation professionally and timely if necessary. Schedule and conduct new resident orientation for all Assisted Living Residents. Provide, plan, schedule, and monitor staff training programs and in-services for the Assisted Living staff including statutory or regulatory required training. Attend all required training, in-service, and staff meetings. Manage and coordinate the work of all nursing and other assisted living staff, communicating pertinent resident health and safety information. This includes overseeing and/or completing staff schedules monthly and daily assignments. Strive to maintain a safe working environment through the prevention of accidents, the preservation of equipment, and the achievement of safe working practices. Ensure compliance with all state regulatory requirements including mandatory reporting to State. Ensure that all concern and incident reports are completed, filed and that follow-up is conducted appropriately. Assist and/or perform in mock surveys. Maintain a positive and professional demeanor toward residents, visitors, families, and co-workers while adhering to all policies, standards and procedures of Company. Require 24/7 on-call clinical support. Perform other duties as assigned. Annual Salary: $123K. Qualifications Managed a clinical team in a senior living or hospital setting. LPN/ LVN or RN license required. Must maintain CEU and other state requirements for the position. Able to manage multiple high-level tasks such as scheduling staff; scheduling care conferences and maintaining communication with staff and families in a professional, positive manner. Managed and developed complex teams. Experience with EMR/EHR clinical documentation systems. Exude energy, passion with a hospitality-focused mindset. A passion for serving seniors and growing teams. Always maintain a positive and friendly demeanor toward the residents and your co-workers. Technology savvy and ability to adapt to a fast-paced environment. Self-awareness organized and time management skills. Must have a valid driver’s license with a clean driving record.
