Description The Clinical Nurse is a Registered Professional Nurse who provides safe, competent quality care based on nursing theory and research to a designated group of patients and significant others. Responsibilities Patient Care 1. Delivers patient care by incorporating the tenets of the professional practice model of Relationship Centered Care and through the application of the nursing process based upon theory, research, evidence-based practice, and approved organizational nursing standards. 2. Assesses and evaluates patient care needs and applies critical thinking skills in patient care management. 3. Integrates relevant assessment and intervention skills in the delivery of nursing care. 4. Collaborates with the interdisciplinary healthcare team in the development, implementation, and evaluation of the plan of care. 5. Prioritizes all aspects of patient care including teaching, rounding, coaching, and planning after hospital care and delegating to others as appropriate. 6. Educates patients and caregivers while anticipating needs and readiness to learn, about their plan of care, transition of care, promotion of health, and prevention of disease. 7. Communicates effectively and professionally with patients, family and all members of the Interdisciplinary Patient Care Team. 8. Manages assignments within the Care Delivery Model of Modified Primary Nursing and demonstrates allocation of material resources effectively. Patient Experience 1. Role models the tenets of Mount Sinai Relationship Centered Care (MSHRCC) through facilitation of the following actions including, but not limited to, Admission Welcome, Bedside shift Report, HELP (High Risk Medications, Equipment, Lines, Drains, Airway, Pain and Plan of Care), AM/PM Care, 5 Minute Sit Down, Purposeful Hourly Rounding, Medication Review, Discharge Wrap Up, and MD/RN Unit Collaboration. 2. Considers the patients values, preferences, expressed needs and knowledge in all aspects of care. 3. Establishes and maintains a therapeutic relationship with the patient and family. 4. Uses evidence-based practices to increase understanding of patient's perceptions of care. 5. Initiates service recovery for patients and family members who have concerns and escalates to Clinical Nurse Manager/Assistant Nurse Manager/Nursing Administrator or Patient Relations as needed. 6. Demonstrates caring, respect, compassion, empathy and active listening through dialogue, body language and actions. 7. Supports staff use of evidence-based practices to increase understanding of patient's perceptions of care. 8. Conducts Purposeful Hourly Rounding (PHR) on assigned patients addressing the 4 Ps. Quality and Safety 1. Practices safe and efficient patient-centered care, identifying and escalating barriers and opportunities for improvement. 2. Demonstrates clinical skills and knowledge crucial to quality and safety in the patient population served. 3. Implements process improvement strategies based on institutional, evidence-based ensure and procedures. 4. Engages in formal and informal peer and institutional review processes. 5. Collaborates with interdisciplinary teams to create and implement quality improvement projects, evidence-based practice activities and nursing research studies. 6. Contributes to quality and safety practices and required compliance measures. 7. Implements principles of high reliability to identify and avoid high risk behaviors to provide a safe environment for patients. 8. Escalates potential safety hazards or gaps from best practice following institutional chain of command protocol. 9. Utilizes institutional information technology by documenting nursing practice to support quality and performance improvement initiatives. Operations 1. Performs charge nurse duties as assigned. 2. Coordinates with the Clinical Coordinator/Clinical Nurse Manager/Assistant Nurse Manager/Nursing Administrator to ensure supplies are appropriately utilized and inventory is sufficient for patient care. 3. Escalates material or staffing resource deficits to charge nurse, Clinical Nurse Manager/Assistant Nurse Manager or Nursing Administrator. 4. Demonstrates patient focused and cost-effective approaches to patient care in terms of equipment, staff, supplies and all other resources. 5. Documents in an accurate and thorough manner in compliance with hospital, regulatory and legal requirements and standards of care. 6. Facilitates patient throughput on a shift basis. Professional Development 1. Contributes to the environment of care to support clinical colleagues, patients and their families and members of the healthcare team. 2. Advances clinical competence in nursing practice to progress from novice to expert. 3. Overview The obstetric unit in Women’s & Infants’ Services is comprised of 16 LDRP (labor/delivery/recovery/postpartum) rooms and an additional 8 post-partum or general care rooms. We provide Level II Maternal Specialty care and deliver approximately 1,750 babies each year under the care of our OB/GYN physician teams from three local clinics. Our Women’s & Infants’ team is supported by a full-time W&I Licensed Social Worker, Inpatient and Outpatient Lactation Support Services and a Women’s Center Outreach team. The Labor & Delivery Registered Nurse (RN) will assume responsibility and accountability for care of the antepartum, intrapartum and postpartum obstetrical patients and their well newborn infants. The L&D RN also performs screening and outpatient care for OB Triage patients along with care to women during their post-partum recovery following vaginal or cesarean delivery. The L&D RN is cross-trained to Mother/Baby care. Job Description The practice of nursing requires specialized knowledge, judgment, and skills to provide care to groups and individuals. The RN utilizes knowledge derived from the principles of biological, physical, behavioral, social, and nursing sciences to assess, plan, implement, and evaluate patient care. All care is provided based on the concepts inherent in the model of care for TCH which promotes an on-going partnership between patients and families and the team of healthcare providers. Responsibilities The Nursing Process Assessment - Demonstrates the nursing skills to complete a holistic assessment of the patient’s physical, psychosocial, spiritual, and education needs utilizing The Christ Hospital Professional Practice Model. Understands signs and symptoms and data related to the patient’s presenting needs and actual or potential nursing diagnosis. Recognizes and appropriately reports to the physician and charge RN pertinent abnormal data and changes in the patient’s condition. Utilizes appropriate handoff methods to collaborate with other healthcare team members to communicate pertinent data regarding patient’s physiological and psychological data. Assesses patient, family, and environment for safety risks, proactively seeks out and performs safety initiatives. Diagnosis and Planning - Utilizes collected data to establish and prioritize a list of actual, and potential, individualized patient problems and needs. Prepares, coordinates, and revises multidisciplinary plan of care in collaboration with the patient and family to be supportive of the TCH model of care/Patient and Family Centered care. For each problem a measurable goal is set. Communicates the plan of care utilizing SBAR and handoff processes with all team members. Establish priorities of care, discusses plan of care during shift report, and participates in care conferences. Initiates discharge planning upon admission, makes appropriate referrals consistent with the anticipated length of stay. Implementation – The method of providing evidence based, quality care is based on assessed needs and standards of care while adhering to hospital policy and procedures to achieve set goals. Performs and delegates nursing actions based on appropriate orders demonstrates critical thinking and accountability for appropriate implementation. Provides ongoing, individualized holistic patient and family education based on needs and plan of care in collaboration with the multidisciplinary team. Demonstrates critical thinking upon review of physician orders, making appropriate referrals, & contacts other health team professionals. Responds to emergency situations that require an immediate, controlled precision in a skillful manner. Implements measures to prevent contamination and/or transmission of disease to include the use of appropriate protective devices and equipment to prevent injury and maintain a safe environment. Evaluation – Continuously evaluates nursing practice related to the plan of care in relation to the standards of care established through evidence-based literature and standards established by professional organization that have been built into order sets in the electronic medical record, the individual plan of care, and patient outcomes and goals. Evaluates effectiveness of nursing care interventions, adjusts or continues with plan of care based on patient and family response. Maintains continuity of care by continuous evaluation during ”handoff” report, rounding, and upon transfers to other specialty units and post care facilities. Documentation Nursing process is accurately and concisely documented in the patient’s electronic medical record reflecting the plan of care as implemented by the patient, family, and multidisciplinary team. Documents information in a timely manner, according to hospital policy, to include the plan of care, accurate updates, teaching and the patients’ and family response to care. A MSN or DNP are not required but supersede the requirement for the BSN. YEARS OF EXPERIENCE: None required REQUIRED SKILLS AND KNOWLEDGE: The RN demonstrates the knowledge, abilities, and skills to provide age and culturally specific patient care and education. Overview The Registered Nurse (RN) provides nursing care to patients utilizing the nursing process and standards of practice for a professional nurse.
