Ensure timely completion and transmission of all assessments, reporting any issues to the Administrator. Assist with the coordination and scheduling of interdisciplinary care plan meetings. Collaborate with outside care team members, including residents' families and representatives. Assist in evaluating residents’ abilities and limitations, ensuring appropriate therapy and activities are scheduled. Contribute to therapy services and assist occupational therapists in carrying out activities. Requirements: Must be a team player with strong collaboration and communication skills. Empathetic and professional when interacting with residents, families, and staff. Must demonstrate integrity and accountability in all work tasks.
