Provides oversight of all patient care services and personnel. Oversight includes making patient and personnel assignments, coordinating patient care, coordinating referrals, assuring patient needs are continually assessed, and assuring the development, implementation, and updates to the individualized patient plan of care. Plans, directs, and participates in delivery of home health services to provide diagnosis-specific patient care, including coordination of services with providers, vendors, or facilities. Ensures the timely start of patient care within recommended regulatory guidelines. Processes and coordinates physician orders, staff assignments, and communications between direct care team members, the patient and their family, the following physician and primary care physician (as appropriate). Reviews and approves plan of care and evaluates proposed changes to the plan of care for clinical appropriateness. Position Summary: Provides high quality nursing care for assigned patients in a scientific and humanistic focus. The Newly Licensed Registered Nurse (NLRN) utilizes a holistic systems approach to protect and promote client wellness and safety in the physiological, psychological, social cultural, developmental and spiritual domains. Nursing interventions are interdisciplinary, age specific and are directed toward optimal wellness. The NLRN is a nurse who is in their first year of nursing practice. Job Description Department: UHC: Peds - CCD - Neurology Job Summary: Coordinator-RN Peds works under the supervision of the Senior Nurse Manager and Clinic Supervisor. Assumes responsibility and accountability for the management of the Pediatric Allergy/Pulmonary, Nephrology, Neurology, Gastroenterology, Infectious Disease, Endocrinology, Rheumatology, and Cardiology, General Pediatrics and/or Pediatric and Adult Medicine patient population on an ongoing basis. Integrates RN nursing processes (assessment, planning, implementation and evaluation) and management (planning, organizing, directing and controlling) to provide services and continuity of care for the patient.
