Registered Nurse (RN) Patient Care Navigator - Transplant Infectious Disease Job Responsibilities: Travel involved: rotation between inpatient and outpatient care at Inova Fairfax Hospital and the Inova Center for Personalized Health, located near Inova Fairfax Hospital. Guides patient and family through the health system from diagnosis, testing, treatment and follow-up care to assist patients with navigating the continuum of care. Coordinates a patient's plan of care with the Transplant Infectious Disease team which may include diagnostic test coordination and specialty medicine coordination and scheduling, patient education and knowledge assessment for complex treatment regimens. Assists patients and families with accessing treatment resources by advising of options and engaging appropriate resources. Manages complex cases effectively and efficiently. Collaborates with multidisciplinary teams to address patient questions and responses about the care process and treatment options. Participates in obtaining quality outcomes such as decreasing patient infection and readmission rates. Develops/maintains reliable systems to document, track and monitor patient data. A two (2) year commitment with a minimum of 36 hours per week (0.90 FTR) to the CVICU is required after the completion of the four (4) month internship program. The initial 4 months will rotate day and night shifts to meet orientation and learning needs. Shift assignment to night shift upon completion of internship. Scheduled paid time off (PTO) will not be granted during the internship program. Responsibilities* The Cardiovascular ICU (CVC 4) is a 24-bed adult intensive care unit caring for patients who have recently undergone cardiac, thoracic, or vascular surgery. This unit specializes in coronary artery bypass, heart and lung transplantation, heart valve repair, ventricular assist devices and esophageal procedures as well as carotid re-vascularization and aneurysm repairs. Staff members collaborate with the Advanced Practice Team and are involved with patient/family conferences, patient care rounds, and referrals. Perform Charge nurse responsibilities as assigned. Skill in communicating in a clear and concise manner with staff involved in critical care, and physicians to ensure the proper care of patients. Skill in reading 12-lead EKG. Skill demonstrated in assigned disciplines such as EP, directly involved in patient care. Ability to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals. Ability to complete routine reports and correspondence. Ability to listen and accurately interpret others’ communication or instructions to take appropriate action. Ability to speak and communicate effectively. Ability to define problems, collect data, establish facts, and draw valid conclusions. Ability to interpret an extensive variety of instructions and deal with several abstract and concrete variables. NICU only: Ability to provide family centered developmentally appropriate care of newborns. Ability to handle the emotional stress associated with caring for a critically ill newborn or neonate.
